摘 要:出血性脑卒中是常见的神经系统急危重症,其致死、致残率高,临床上并无确切有效的干预措施改善此类患者的预后。出血性脑卒中的病理生理学机制复杂,不仅包括血肿和脑水肿引起的原发性损伤,而且还可因颅内压增高、能量代谢异常、神经炎症和兴奋性毒性等造成继发性损伤。亚低温在神经重症医学领域中应用日渐广泛,基础研究显示其具有广泛脑组织保护和降低颅内压的作用,并对各种类型脑损伤的病理生理学变化具有抑制作用。因此,本文就亚低温在出血性脑卒中保护的作用机制、降温时机和不良反应等的最新研究进展进行简要综述。Hemorrhagic stroke is a common nervous system emergency and severe disease,with high mortality and disability rate.There is no effective intervention to improve the prognosis of these patients.The pathophysiological mechanism of hemorrhagic stroke is complicated,not only including primary injury caused by hematoma and edema,but also secondary damage caused by abnormal energy metabolism,neuro-inflammatory reaction and excitotoxicity.Mild hypothermia is widely used in the field of neurosurgical intensive medicine.Basic researches have showed that mild hypothermia has extensive brain protection and reduction of intracranial pressure,and also has a protective effect through the various pathway of secondary injury.Therefore,this review synthesizes the latest research progress of mild hypothermia in the mechanism of cerebral protection,coolingtiming and adverse reactions of hemorrhagic stroke.
