骨皮质剥脱术联合锁定接骨板内固定术治疗四肢长骨骨不连的临床效果

摘　　要：目的研究骨皮质剥脱术联合锁定接骨板内固定术治疗四肢长骨骨不连的临床效果。方法选取2016年4月1日～2017年4月1日我院收治的90例四肢长骨骨不连患者作为研究对象,根据治疗方法的不同分为两组,每组各45例。对照组实施骨皮质剥脱术,观察组实施骨皮质剥脱术联合锁定接骨板内固定术。比较两组患者的临床相关指标、关节活动度、肌力、日常生活能力评分、临床效果及并发症的发生情况。结果观察组患者的手术时间、住院时间、骨折愈合时间均短于对照组,出血量少于对照组,差异有统计学意义(P<0.05);观察组治疗后的关节活动度、肌力评分、日常生活能力评分及治疗优良率均高于对照组,差异有统计学意义(P<0.05);观察组患者的并发症总发生率低于对照组,差异有统计学意义(P<0.05)。结论骨皮质剥脱术联合锁定接骨板内固定术应用于四肢长骨骨不连患者的治疗中,有效性和安全性均较高。Objective To study the clinical effect of cortical exfoliation combined with locking plate internal fixation in the treatment of long bone nonunion of extremities.Methods A total of 90 cases patients with long bone nonunion of the extremities treated in our hospital from April 1,2016 to April 1,2017 were selected as the subjects,and divided into two groups according to different treatment methods,with 45 cases in each group.Patients in the control group were treated with cortical exfoliation,and patients in the observation group were treated with cortical exfoliation combined with locking plate internal fixation.The clinically relevant indicators,joint mobility,muscle strength,daily living ability score,clinical effect and complications between the two groups were compared.Results The operation time,hospitalization time and fracture healing time in the observation group were shorter than those in the control group,and the amount of bleeding was less than that in the control group,and the differences were statistically significant(P<0.05).The joint mobility,muscle strength scores,daily living ability score data and treatment excellent rate in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).The total complication rate in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Cortical exfoliation combined with locking plate internal fixation is effective and safe in the treatment of patients with long bone nonunion of extremities.
