摘　　要：目的探讨血液生化指标连续检测在创伤骨折中的应用价值。方法选取2015年1月～2018年1月我院骨科收治的75例创伤性骨折患者作为研究对象,根据AIS-ISS90分级法分为重度组(35例)与轻度组(40例),动态监测降钙素原(PCT)、C反应蛋白(CRP)、高密度脂蛋白胆固醇(HDL-C)、肌酸激酶(CK)及肌酸激酶同工酶(CKMB)的表达情况。结果创伤后1 d,重度组患者血清中的PCT、CRP指标均明显高于轻度组(P<0.05);创伤后7 d,两组患者血清中的PCT、CRP均明显下降(P<0.05),重度组的PCT、CRP仍高于轻度组(P<0.05);创伤后1 d,重度组患者血清中的HDL-C指标明显低于轻度组(P<0.05);创伤后7 d,两组患者血清中的HDL-C均明显上升(P<0.05),重度组的HDL-C仍低于轻度组(P<0.05);创伤后1 d,重度组患者血清中的CK、CK-MB指标均明显高于轻度组(P<0.05);创伤后7 d,两组患者血清中的CK、CK-MB均明显下降(P<0.05),重度组CK、CK-MB仍高于轻度组(P<0.05)。结论 PCT、CRP、HDL-C、CK及CK-MB在创伤性骨折患者中的动态波动具有一定的规律性,同时对于相关血清学指标的连续性监测,能够为疾病的病情和临床转归提供参考。Objective To explore the application value of blood biochemical in traumatic fracture.Methods A total of 75 cases fracture patients treated in our department of orthopedics from January 2015 to January 2018 were selected as the subjects,and divided into severe group(35 cases)and mild group(40 cases)according to the AIS-ISS90 classification.Dynamic monitoring of the expression of procalcitonin(PCT),C reactive protein(CRP),high density lipoprotein cholesterol(HDL-C),creatine kinase(CK)and creatine kinase(CK-MB)were conducted.Results The 1 d after trauma,PCT and CRP in the severe group were higher than the mild group(P<0.05).7 d after trauma,the PCT and CRP significantly decreased in the two groups(P<0.05),while the PCT,CRP in the severe group were still higher than that in the mild group(P<0.05).1 d after trauma,HDL-C in the severe group was significantly lower than that in the mild group(P<0.05).7 d after trauma,the HDL-C significantly increased in the two groups(P<0.05),while HDL-C in the severe group was still lower than that in the mild group(P<0.05).1 d after trauma,CK and CK-MB index in severe group were significantly higher than those in the mild group(P<0.05).7 d after trauma,the CK and CK-MB significantly decreased in the two groups(P<0.05),while CK,CK-MB in severe group was still higher than those in the mild group(P<0.05).Conclusion The dynamic fluctuations of PCT,CRP,HDL-C,CK and CK-MB in traumatic fracture patients have certain regularity.Meanwhile,continuous monitoring of related serological indicators can provide references for disease condition and clinical outcome.
