综合性围术期护理在手部组织缺损患者中的应用效果

摘　　要：目的探讨综合性围术期护理在手部组织缺损患者中的应用效果。方法选取2014年1月～2017年1月我院整形烧伤手外科收治的手部组织缺损82例患者作为研究对象,根据其入院序号奇偶数分为对照组和观察组,每组各41例。全部患者经游离肱骨皮瓣结合同种异体骨移植修复,对照组给予常规护理干预方案,观察组在此基础上行综合性围术期护理干预,观察比较两组手部功能恢复优良率、疼痛情况、修复满意情况和并发症发生率。结果随访1年,观察组手部功能恢复优良率为85.36%,明显高于对照组的65.86%(P<0.05);观察组疼痛评分低于对照组,外观满意度评分高于对照组(P<0.05);护理6个月后,观察组血管脱落、吻合口漏血、撕裂发生率分别为2.44%、4.88%、2.44%,均低于对照组的14.63%、19.51%和17.07%,差异均有统计学意义(P<0.05)。结论在手部组织缺损患者游离肱骨皮瓣结合同种异体骨移植修复中,实施综合性围术期护理干预效果理想,推荐临床推广应用。Objective To investigate the application effect of comprehensive perioperative nursing in patients with hand tissue defects.Methods From January 2014 to January 2017,82 patients with hand tissue defects admitted to our department were selected as the study subjects.According to their odd/even admission,they were divided into control group and observation group,41 cases in each group.All participants were performed with free tibial flap combined with allogeneic bone graft.In the control group,routine nursing intervention was used,while in the observation group,on the basis of the control group,comprehensive perioperative nursing intervention was added.The excellent and good rate of hand function recovery,pain,repair satisfaction,and incidence of complications were observed and compared between two groups.Results Within 1 year of follow-up,the excellent and good rate of hand function recovery in the observation group was 85.36%,significantly higher than that in the control group accounting for 65.86%(P<0.05).Compared with the control group,the pain score was lower and the appearance satisfaction was higher during follow-up(P<0.05).After 6-month nursing,the incidence rates of vascular shedding,anastomotic leakage,and tearing in the observation group were 2.44%,4.88%,and 2.44%,which were lower than those in the control group of 14.63%,19.51%,and 17.07%,the differences were statistically significant(P<0.05).Conclusion In the process of repairing free tibial flap combined with allogeneic bone graft in patients with hand tissue defects,comprehensive perioperative nursing intervention is effective and recommended.
