摘 要:目的探讨原发肿瘤体积在调强放射治疗下鼻咽癌的预后价值。方法回顾性分析2004年1月—2007年12月500例初诊并排除远转的调强放射治疗的鼻咽癌患者。采用Kaplan-Meier、Log-rank、Cox回归模型进行预后分析。结果 5年无局部复发率、无远处转移率、无病生存率、总生存率分别为90.6%、83.8%、71.5%和79.3%。年龄(P<0.001)、N分期(P=0.001)、原发肿瘤体积(P=0.001)为不良预后因子。T分期和同步化疗不是显著的预测因子。结论 IMRT能获得较好的鼻咽癌治疗效果。原发肿瘤体积>50 mL可能可以作为独立的预后预测因子。Objective To investigate the prognostic value of primary tumor volume in nasopharyngeal carcinoma under intensity modulated radiation therapy.Methods A retrospective analysis of 500 patients with nasopharyngeal carcinoma who were newly diagnosed and excluded from intensity-modulated radiation therapy from January 2004 to December 2007 were retrospectively analyzed.Prognostic analysis was performed using Kaplan-Meier,Log-rank,and Cox regression models.Results There were no local recurrence rate,no distant metastasis rate,disease-free survival rate and overall survival rate of 90.6%,83.8%,71.5% and 79.3%,respectively.Age (P<0.001),N stage (P=0.001),and primary tumor volume (P=0.001) were poor prognostic factors.T staging and concurrent chemotherapy were not significant predictors.Conclusion IMRT can obtain better therapeutic effect on nasopharyngeal carcinoma.A primary tumor volume >50 mL may be an independent predictor of prognosis.
