摘 要:目的评价对比膝关节外伤应用MR扫描技术与CT的价值。方法随机选取该院2016年1月—2018年5月收治的膝关节外伤患者200例为对象进行研究,回顾性分析其全部的临床资料,所有患者均实施MR扫描技术与CT诊断。比较两种方法对韧带损伤、关节腔积液、骨髓水肿、半月板软骨破损、关节面软骨损伤、轻微骨折、严重骨折等的检出阳性率以及比较两组患者前交叉韧带(ACL)及后交叉韧带(PCL)的测量值。结果该组200例患者采用MR扫描技术检出韧带损伤、关节腔积液、骨髓水肿、半月板软骨破损、关节面软骨损伤、轻微骨折、严重骨折等的阳性率分别为28.5%、100.0%、35.0%、34.5%、9.0%、26.0%、14.0%,而CT诊断各损伤类型的阳性率分别为的0.0%、28.0%、0.0%、0.0%、0.0%、3.0%、5.5%,MR检出阳性率较CT诊断均明显较高,且差异有统计学意义(P<0.05)。观察组ACL测量值的宽度(10.1±0.6)mm、厚度(5.8±0.6)mm以及PCL测量值的宽度(11.2±0.5)mm、高度(5.4±0.3)mm等与对照组(9.8±0.9)mm、(5.6±0.9)mm、(11.1±0.4)mm、(5.2±0.8)mm比较差异无统计学意义(t=1.032、1.001、1.364、1.654,P>0.05)。结论与CT诊断相比,膝关节外伤采用MRI扫描技术诊断的效果更为显著,即能达到较高的准确性,且可为患者预后判断提供可靠依据,因此值得临床应用推广。Objective To evaluate the value of MR scanning technique and CT in the treatment of knee joint trauma.Methods A total of 200 patients with knee trauma who were admitted to the hospital from January 2016 to May 2018 were randomly selected for retrospective analysis of all clinical data.All patients underwent MR scanning and CT diagnosis.Comparing the two methods for the detection of ligament injury,joint effusion,bone marrow edema,meniscus cartilage damage,articular cartilage damage,minor fractures,severe fractures,etc.,and comparing the anterior cruciate ligament (ACL) and posterior measured value of the cruciate ligament (PCL).Results The positive rate of ligament injury,joint effusion,bone marrow edema,meniscus cartilage damage,articular cartilage injury,minor fracture and severe fracture in the 200 patients in this group were 28.5% and 100.0%,35.0%,34.5%,9.0%,26.0%,14.0%,and the positive rates of CT diagnosis of each type of injury were 0.0%,28.0%,0.0%,0.0%,0.0%,3.0%,5.5%,respectively.The positive rate was significantly higher than that of CT,and the difference was statistically significant(P<0.05).The width of the observation group ACL measurement value (10.1±0.6)mm,thickness (5.8±0.6)mm,width of PCL measurement value (11.2±0.5)mm,height (5.4±0.3) mm,etc.and the control group (9.8±0.9)mm,(5.6±0.9)mm,(11.1±0.4)mm,and (5.2±0.8)mm (t=1.032,1.001,1.364,1.654,P>0.05).Conclusion Compared with CT diagnosis,the effect of MRI scanning technique on knee joint trauma is more significant,which can achieve higher accuracy and provide a reliable basis for patient prognosis.Therefore,it is worthy of clinical application.
