预混胰岛素联合二甲双胍对新诊断2型糖尿病患者胰岛功能恢复的影响

摘　　要：目的:分析预混胰岛素联合二甲双胍对新诊断2型糖尿病患者胰岛功能恢复的影响。方法:90例新诊断2型糖尿病患者按简单随机数字表法分为48例对照组和42例研究组。对照组采用二甲双胍治疗,研究组在对照组基础上采用预混胰岛素治疗。比较两组血糖达标时间,治疗前、治疗8周及12周后血糖代谢指标,胰岛功能,肝肾功能,和不良反应情况。结果:研究组血糖达标时间高于对照组(7.41±1.02)和(11.78±1.90) d,比较有统计学差异(P<0.05)。治疗治疗8周及12周后,两组血糖代谢指标均下降,研究组低于对照组(P<0.05)。研究组空腹胰岛素(FINS)、胰岛素抵抗指数(HOMA-IR)低于对照组、胰岛β细胞功能(HOMA-β)高于对照组,比较有统计学差异(P<0.05)。两组治疗前后肝肾功能比较无统计学差异(P>0.05)。两组均有低血糖、胃肠道反应发生,组间总不良反应发生率比较无统计学差异(P>0.05)。结论:预混胰岛素联合二甲双胍作为新诊断2型糖尿病患者的治疗方案是有效安全的,可有效控制血糖,促进胰岛素功能恢复。Objective:To analyze the effect of premixed insulin combined with metformin on islet function recovery in newly diagnosed type 2 diabetes mellitus.Methods:90 patients with newly diagnosed type 2 diabetes who according to simple random number table method were divided into 48 control group and 42 research group.The control group was treated with metformin,and the research group was treated with premixed insulin based on the control group.Then blood glucose compliance time,blood glucose metabolism indicators,islet function,liver and kidney function before and after treatment after 8 weeks and 12 weeks after treatment,and adverse reaction in both group were compared.Results:blood glucose compliance time of research group was higher than that in the control group(7.41±1.02)and(11.78±1.90)days(P<0.05).After 8 weeks and 12 weeks treatment,the blood glucose metabolism indexes in both groups was decreased,and the research group was lower than the control group(P<0.05).The fasting insulin(FINS),insulin resistance index(HOMA-IR)in research group were lower than the control group,while the islet beta cell function(HOMA-β)was higher than the control group,there was a statistically significant difference(P<0.05).liver and kidney function before and after treatment in both group was no statistically significant difference(P>0.05).Both groups had hypoglycemia and gastrointestinal reactions,the incidence of total adverse reactions in both groups was no statistically significant difference(P>0.05).Conclusion:Insulin pretreatment combined with metformin is effective and safe for the new diagnosis of type 2 diabetes,which can effectively control blood glucose and promote the recovery of insulin function.
