有创颅内压监测对重型颅脑损伤的疗效分析

摘　　要：目的研究分析重型颅脑损伤患者术后治疗中应用有创颅内压监测的临床价值。方法选取2014年6月—2018年4月间于我院神经外科治疗的86例重型颅脑损伤患者作为研究对象,将所有患者随机分为实验组和对照组,每组43例,实验组行有创颅内压监测,对照组给予常规颅内压评价方法,评估全部患者的格拉斯哥预后评分(GOS),进行预后对比,记录并对比两组患者术后应用甘露醇的剂量及时间。结果不同方式颅内压监测,实验组预后良好率显著高于对照组,差异有统计学意义(P<0.05)。实验组患者术后应用甘露醇剂量显著少于对照组,甘露醇应用时间显著短于对照组,差异有统计学意义(P <0.05)。结论重型颅脑损伤患者术后治疗中应用有创颅内压监测能够显著改善患者预后状况,降低甘露醇的使用时间及剂量,效果显著,值得临床推广应用。Objective To study the clinical value of invasive intracranial pressure monitoring in postoperative treatment of patients with severe craniocerebral injury.Methods Eighty-six patients with severe craniocerebral injury who were treated with neurosurgery in our hospital from June 2014 to April 2018 were enrolled.All patients were randomly divided into two groups,the experimental group and the control group.43 participants were in each group.The experimental group was monitored by invasive intracranial pressure,and the control group was assessed by routine intracranial pressure assessment method.The Glasgow Outcome Scale(GOS)score was evaluated after operation in all patients,and the prognosis was compared.The dose and time of mannitol application were recorded and compared between the two groups.Results The good prognosis rate of the experimental group was significantly higher than that of the control group(P<0.05).The dosage of mannitol in the experimental group was significantly less than that in the control group,and the duration of mannitol application was significantly shorter than that in the control group(P<0.05).Conclusion Invasive intracranial pressure monitoring can significantly improve the prognosis of patients with severe craniocerebral injury and reduce the use of mannitol time and dose.Its effect is significant.It is worthy of clinical application.
