实时血糖监测辅助持续皮下胰岛素输注联合西格列汀治疗脆性糖尿病的疗效及安全性

摘　　要：目的探究实时血糖监测辅助持续皮下胰岛素输注(continuous subcutaneous insulin infusion,CSII)联合西格列汀治疗脆性糖尿病的疗效及安全性。方法收集2013年10月至2017年10月本院收治的90例脆性糖尿病患者进行前瞻性研究,根据治疗方式不同将其分为研究组(48例)和对照组(42例),对照组患者采取CSII联合西格列汀治疗,研究组患者采用实时血糖监测辅助CSII联合西格列汀治疗。比较两组患者治疗前后血糖指标[平均血糖(mean blood glucose,MBG),空腹血糖(fasting plasma glucose,FPG)]、血糖波动指标[平均血糖波动幅度(mean amplitude of plasma glucose,MAGE),高血糖曲线下面积(AUC_(10),血糖水平≥10 mmoL/L的AUC)、低血糖曲线下面积(AUC_(3.9),血糖水平≤3.9 mmoL/L的AUC)]及低血糖事件发生率。结果治疗后,两组患者MBG和FPG水平、MAGE、AUC_(10)及AUC_(3.9)均显著低于本组治疗前(P均<0.05),且研究组患者上述指标水平均显著低于对照组(P_均<0.05)。研究组患者低血糖事件发生率显著低于对照组(P <0.05)。结论实时血糖监测辅助CSII联合西格列汀可有效改善脆性糖尿病患者的血糖水平,减少血糖波动,明显减少低血糖事件的发生。Objective To explore the efficacy and safety of real-time blood glucose monitoring assisted continuous subcutaneous insulin infusion(CSII)combined with Sitagliptin in the treatment of brittle diabetes.Method A prospective study was conducted on 90 patients with brittle diabetes admitted to our hospital from October 2013 to October 2017,they were divided into study group(n=48)and control group(n=42)according to the treatment method.Among them,control group patients were treated with CSII combined with Sitagliptin,study group patients were treated with real-time blood glucose monitoring and CSII combined with Sitagliptin.The blood glucose indexes[mean blood glucose(MBG),fasting plasma glucose(FPG)],blood glucose fluctuation indexes[mean amplitude of plasma glucose(MAGE),AUC10 and AUC3.9]before and after treatment and hypoglycemia events were compared between the two groups.Result After treatment,the MBG and FPG levels,MAGE,AUC10 and AUC3.9 of the two groups were significantly lower than those before treatment(Pall<0.05),and the above indexes of study group were significantly lower than those of control group(Pall<0.05).The incidence of hypoglycemia in study group was significantly lower than that in control group(P<0.05).Conclusion Real-time blood glucose monitoring and CSII combined with Sitagliptin can effectively improve brittle diabetes patient's blood glucose levels,stabilize blood sugar reduction fluctuations,and significantly reduce the incidence of hypoglycemia.
