V1RP-间期值在慢-快型房室结折返性心动过速中的诊断价值

摘　　要：目的探讨V1RP-间期值在慢-快型房室结折返性心动过速(AVNRT)中的诊断价值。方法选取2014年1月至2017年12月行射频消融术治疗的慢-快型AVNRT组262例患者和顺向型AVRT组285例患者,观察V1RP-间期值对于诊断慢-快型AVNRT的灵敏度、特异度、最佳临界值。结果V1RP-间期<70ms时诊断慢-快型AVNRT的灵敏度、特异度为75.2%、97.2%;由Youden指数计算V1RP-间期临界值为89.5ms,据此诊断慢-快型AVNRT灵敏度为98.9%,特异度为86.6%。V1RP-间期≥90ms诊断顺向型AVRT的灵敏度、特异度分别为80.7%、98.9%。结论诊断慢-快型AVNRT的最优V1RP-间期切点为<90ms。Objective To investigate the diagnostic value of V1RP-interval in slow-fast atrioventricular node reentrant tachycardia(AVNRT).Methods 262 cases of slow-fast AVNRT and 285 cases of orthodromic AVRT treated by radiofrequency ablation from January 2014 to December 2017 were enrolled.The sensitivity and specificity of different V1RP-interval in the diagnosis of slow-fast AVNRT were analyzed and the optimal cutoff value of V1RP-interval was explored.Results In the diagnosis of slow-fast AVNRT,the sensitivity and specificity of V1RP-interval<70ms were 75.2%and 97.2%,respectively,while the sensitivity and specificity of V1RP-interval<89.5ms estimated from Youden index were 98.9%and 86.6%,respectively.The sensitivity and specificity of V1RP-interval≥90ms in diagnosing orthodromic atrioventricular reentrant tachycardia were 80.7%and 98.9%,respectively.Conclusion The optimal cutpoint of V1RP-interval in the diagnosis of AVNRT is<90ms.
