摘　　要：目的探讨Lorenz-RR间期散点图在快速诊断心律失常中的应用价值。方法选取545例患者的Lorenz-RR间期散点图,根据诊断结果分为窦性心律、室上性早搏、室性早搏、心房颤动、心房扑动5组,比较动态心电图诊断结果与Lorenz-RR间期散点图诊断结果,分析二者的符合率及各组B线斜率。结果利用Lorenz-RR间期散点图与24 h动态心电图分析心律失常,二者符合率较高。室上性早搏B线斜率与室性早搏、心房颤动、心房扑动之间存在显著差异(P<0.001);室性早搏的B线斜率与心房颤动、心房扑动之间存在显著差异(P<0.001);心房颤动与心房扑动的B线斜率无明显差异(P>0.05)。结论利用Lorenz-RR间期散点图诊断心律失常速度快、符合率高,尤其对室上性早搏、室性早搏、房颤、房扑等具有良好的临床应用价值。Objective To discuss the application value of Lorenz-RR interval scatterplot in rapid diagnosis of arrhythmia.Methods A total of 545 patients'Lorenz-RR interval scatterplot were selected,which were divided into 5 groups(sinus rhythm,supraventricular premature beats,ventricular premature beats,atrial fibrillation and atrial flutter)by the diagnostic results.The diagnostic results of dynamic electrocardiogram and Lorenz-RR interval scatterplot were compared,the coincidence rate and B-line slope of each group were analyzed.Results Lorenz-RR interval scatterplot and 24-hour ambulatory electrocardiogram were used to analyze arrhythmia,and the coincidence rate between them was higher.The B-line slope of supraventricular premature beats was significantly different from that of ventricular premature beats,atrial fibrillation and atrial flutter(P<0.001).The B-line slope of premature ventricular beats was significantly different from that of atrial fibrillation and atrial flutter(P<0.001).There was no significant difference in B-line slope between atrial fibrillation and atrial flutter(P >0.05).Conclusion Lorenz-RR interval scatterplot is a fast and accurate method for the diagnosis of arrhythmia,especially for supraventricular premature beats,ventricular premature beats,atrial fibrillation and atrial flutter.
