摘  要:目的探讨miR-126和s VCAM-1在2型糖尿病肾病患者血清中的表达及临床意义。方法选取内分泌科住院的2型糖尿病患者256例,其中2型糖尿病无肾病组135例,2型糖尿病肾病组121例,同时选取体检健康者48例为对照组,检测血清中miR-126和s VCAM-1相对表达量,进行相关分析。结果与对照组比较,2型糖尿病无肾病组和2型糖尿病肾病组BMI差异无统计学意义(F=0. 78,P> 0. 05); 2型糖尿病无肾病组和2型糖尿病肾病组FBG、Hb A1c、24hpro、Upro/Ucr、s VCAM-1 mRNA相对表达量均增高,e GFR和miR-126mRNA相对表达量降低,差异有统计学意义(F=245. 11、130. 67、115. 28、127. 88、1230. 42、507. 56、652. 20,P <0. 05)。3组血清中miR-126与BMI相关性(r=-0. 031、-0. 039、-0. 028,P> 0. 05),与FBG、Hb A1c、24hpro、Upro/Ucr呈负相关(r=-0. 341、-0. 402、-0. 516、-0. 598、-0. 392、-0. 410、-0. 441、-0. 557、-0. 530、-0. 442、-0. 528、-0. 465,P <0. 05),与e GFR呈正相关(r=0. 377、0. 382、0. 395,P> 0. 05)。3组血清中s VCAM-1与BMI无相关性(r=0. 250、0. 231、0. 260,P> 0. 05),与FBG、Hb A1c、24hpro、Upro/Ucr呈正相关(r=0. 441、0. 502、0. 566、0. 538、0. 390、0. 425、0. 541、0. 507、0. 430、0. 440、0. 528、0. 665,P <0. 05),与e GFR呈负相关(r=-0. 477、-0. 482、-0. 400,P> 0. 05)。3组血清中miR-126和s VCAM-1均呈负相关(r=-0. 347、-0. 371、-0. 382,P <0. 05)。结论血清中miR-126相对表达量偏低或s VCAM-1相对表达量偏高是影响2型糖尿病肾脏疾病患者的危险性因素,可能在2型糖尿病肾病发生发展过程中发挥重要作用。Objective To investigate the expression and clinical significance of miR-126 and sVCAM-1 in patients with type 2 diabetic nephropathy.Methods A total of 256 patients with type 2 diabetes mellitus(T2DM)who were admitted and treated in our hospital from October 2015 to October 2017 were divided into T2DM group(n=135),T2DKD group(n=121),at the same time,the other 48 healthy subjects were enrolled as control group.The general clinical data and relative expression levels of serum miR-126 and sVCAM-1 were observed and compared among the thre groups.Results As compared with that in control group,there was no significant difference in BMI between T2DM group and T2DKD group(F=0.78,P >0.05).The relative expression levels of FBG,HbA1c,24hpro,Upro/Ucr and sVCAM-1 mRNA in T2DM group and T2DKD were significantly increased,and the relative expression levels of eGFR and miR-126 mRNA were significantly decreased(P<0.05).The serum levels of miR-126 were not correlated with BMI(r=-0.031,-0.039,-0.028,respectively,P >0.05),however,which were negatively correlated with those of FBG,HbA1c,24hpro and Upro/Ucr(P<0.05),moreover,which were positively correlated with those of eGFR(P >0.05).In addition the serum levels of sVCAM-1 in the three groups were not correlated with those of BMI(P >0.05),however,which were positively correlated with those of FBG,HbA1c,24hpro and Upro/Ucr(P<0.05),and which were negatively correlated with those of eGFR(P >0.05).The serum levels of miR-126 and sVCAM-1 in the three groups were negatively correlated each other(P<0.05).Conclusion The relatively low expression of miR-126 or relatively high expression of sVCAM-1 is one of risk factors for patients with type 2 diabetic nephropathy,which suggests that miR-126 and sVCAM-1 may play an important role in the pathogenesis and development of type 2 diabetic nephropathy.
