息肉状脉络膜血管病变患者外周血单核细胞CD11b和CD200的表达及意义

摘　　要：目的检测息肉状脉络膜血管病变(PCV)患者外周血单核细胞CD11b、CD200的表达水平并探究其与PCV患者临床特征的关系。方法收集2015年10月至2017年10月诊治的PCV患者86例(103眼)为研究对象(PCV组),选取同期健康体检者80例作为对照组。检测各受试者外周血中CD11b+、CD200+、CD11b+CD200+单核细胞比例,裂隙灯显微镜检查PCV患者玻璃体积血情况,眼底彩色照相检查PCV患者眼底病变情况,荧光素眼底血管造影(FFA)检查PCV患者眼中血管病变情况,吲哚青绿血管造影(ICGA)检查PCV患者病变位置并进行比较。结果 PVC患者外周血中CD11b+、CD200+、CD11b+CD200+单核细胞百分比均显著高于健康体检者,差异有统计学意义(P <0. 05); PVC患者外周血中CD11b+单核细胞百分比与患者视网膜下出血情况、病变区域大小、息肉状病灶数及疾病分型有关(P <0. 05),与患者年龄、性别、体重指数、吸烟情况、饮酒情况、病变眼数目及病变位置无关(P> 0. 05)。PVC患者外周血中CD200+、CD11b+CD200+单核细胞百分比与患者视网膜下出血情况、病变区域大小、息肉状病灶数及疾病分型、年龄、性别、体重指数、吸烟情况、饮酒情况、病变眼数目及病变位置均无关(P> 0. 05)。CD11b+单核细胞比例与CD200+、CD11b+CD200+单核细胞比例无明显相关性(P> 0. 05); CD200+单核细胞比例与CD11b+CD200+单核细胞比例无明显相关性(P> 0. 05)。结论 PCV患者外周血中CD11b+单核细胞比例显著增加,与患者视网膜下出血情况、病变区域大小、息肉状病灶数及疾病分型有关,可能作为评估PCV患者疾病严重程度的生物指标。Objective To detect the expression levels of CD11b and CD200 mononuclear cells in peripheral blood of patients with polypoid choroidal vasculopathy(PCV),and to explore its relationship with clinical characteristics of patients with PCV.Methods A total of 86 patients(103 eyes)with PCV who were diagnosed and treated in our hospital from October 2015 to October 2017 were enrolled as PCV group,the other 80 healthy persons who underwent health examination in the same period were enrolled as control group.The proportions of CD11b^+,CD200^+and CD11b^+CD200^+mononuclear cells in peripheral blood of the subjects were detected,and the slit lamp microscope was used to examine the blood accumulation of vitreous blood in patients with PCV,the fundus lesions of PCV patients were examined by fundus color photography,moreover,fluorescein fundus angiography(FFA)was used to examine the vascular lesions in the eyes of PCV patients,and indocyanine green angiography(ICGA)was used to examine and compare the location of pathological changes in patients with PCV.Results The percentages of CD11b^+,CD200^+and CD11b^+CD200^+mononuclear cells in PVC group were significantly higher than those in control group(P<0.05).The percentage of CD11b^+mononuclear cells in peripheral blood of patients with PVC was related to the subretinal hemorrhage,the size of lesion area,the number of polypoid lesions and the classification of disease(P<0.05),however,which was not related to patient’s age and sex,physique ratio,smoking,drinking,the number of diseased eyes,and the location of the lesion(P >0.05).In addition the percentages of CD200^+,CD11b^+CD200^+mononuclear cells in peripheral blood of patients with PVC were not correlated with the subretinal hemorrhage,the size of lesion area,the number of polypoid lesions and the classification of disease,age,sex,physique ratio,smoking,drinking,the number of diseased eyes,and the location of the lesion(P >0.05).The proportion of CD11b^+mononuclear cells was not significantly correlated with the proportion of CD
