心脉隆注射液治疗老年慢性阻塞性肺疾病合并心力衰竭临床评价

摘　　要：目的探讨心脉隆注射液治疗慢性阻塞性肺疾病(COPD)合并心力衰竭老年患者的疗效及对血浆超敏C反应蛋白(hs-CRP)与N-末端脑钠肽前体(NT-proBNP)的影响。方法选取医院2016年1月至2018年1月收治的COPD合并心力衰竭老年患者92例,随机分为对照组和观察组,各46例。对照组患者给予常规治疗,观察组患者加用心脉隆注射液治疗。结果治疗后,两组患者白细胞介素10(IL-10)、第1秒用力呼气容积(FEV_1)、用力肺活量(FVC)、第1秒用力呼气容积占用力肺活量百分比(FEV_1/FVC)和最大呼气峰流速(PEFR)水平均升高,血浆hs-CRP、NT-proBNP、白细胞介素18(IL-18)水平均降低,且观察组变化幅度大于对照组(P <0.05);治疗后,观察组心功能疗效明显优于对照组(P <0.05);观察组患者初次使用心脉隆注射液时出现轻微恶心1例,穿刺处有轻度刺痒感2例,轻度心悸3例,减慢滴速后不良反应均消失,后续用药无上述现象发生。结论心脉隆注射液能有效改善COPD合并心力衰竭老年患者的肺功能和心功能,缓解炎性反应,且无严重不良反应。Objective To investigate the clinical effect of Xinmailong Injection in the treatment of elderly patients with chronic obstructive pulmonary disease(COPD)complicated with heart failure and its effect on plasma hs-CRP and NT-proBNP.Methods Totally 92 elderly patients with COPD complicated with heart failure admitted to our hospital from January 2016 to January 2018 were selected and randomly divided into the control group and the observation group,46 cases in each group.The patients in the control group were given routine treatment,on this basis,the patients in the observation group were given Xinmailong Injection.Results After treatment,the levels of IL-10,FEV1,FVC,FEV1/FVC and PEFR in the two groups were increased,while the levels of hs-CRP,NT-proBNP and IL-18 in the two groups were decreased,and the changes in the observation group were more significant than those in the control group(P<0.05).After treatment,the effective rate of cardiac function in the observation group was significantly better than that in the control group(P<0.05).When the patients in the observation group used Xinmailong Injection for the first time,there was 1 case of slight nausea,2 cases of mild itching at the puncture site and 3 cases of mild palpitation.The adverse reactions disappeared after slowing down the drip rate,and no such phenomenon occurred in the follow-up medication.Conclusion Xinmailong Injection can effectively improve the pulmonary function and cardiac function of patients with COPD complicated with heart failure,alleviate inflammatory reaction and has no serious adverse reactions.
