百岁老人住院的病因和临床特点

摘　　要：目的调查分析住院百岁老人入院病因、临床特点、住院现状、疾病转归和死亡原因。方法回顾性分析武汉大学中南医院老年病科住院的百岁老人一般情况、入院病因、疾病种类、服药数量、住院天数、住院次数和疾病转归。结果 13例住院百岁老人年龄为(100. 92±1. 32)岁;其中男性11例,女性2例。首要入院病因为肺部感染,占61. 54%;死亡率达38. 46%,死亡原因以重症肺部感染合并多脏器功能衰竭为主,约占60%。所有百岁老人均合并多种慢性病,平均(6. 0±2. 31)种/人,所患病种最高为高血压(10例,76. 92%);其次老年痴呆和低蛋白血症(各9例,69. 23%);第三为慢性阻塞性肺疾病、心功能不全和肾功能不全(各6例,46. 15%)。11例百岁老人住院期间口服药物为(6. 55±3. 64)种/人,2例因不能进食未口服药。结论住院百岁老人普遍存在多病共存、住院时间长、多重用药的特点。入院病因和死亡病因主要为肺部感染,多脏器功能障碍。Objective To investigate the hospitalization situation and the clinical characteristics of centenarians inpatients(≥100 years)and evaluate their prognosis.Methods All centenarians inpatients came from department of geriatrics of Zhongnan hospital of Wuhan university.Then a retrospective study was performed on their medical records and clinical data,such as age,sex,doctor-visit causes,comorbidity,drug application,hospitalization time,the cause of death and prognosis.Results The study comprised 13 subjects[(100.92±1.32)year,including 11 men and 2 women].Main hospitalization causes were pneumonia 61.54%.There were 5 patients dead during hospitalization,the causes of death were severe pulmonary infection(60%).All centenarians were accompanied by a variety of chronic diseases.The top three admitting diagnoses were hypertension(76.92%),dementia(69.23%)and hypoproteinemia(69.23%),chronic obstructive pulmonary disease 46.15%,cardiac insufficiency 46.15%and renal inadequacy 46.15%.The average number of oral drugs taken by 11 centenarians during hospitalization was(6.55death group were compa,and others took no drugs.Conclusions The patients over 100 year old is characterized by multiple disease coexistence,long hospital stay and multiple medications.The cause of hospitalization and the cause of death are mainly pulmonary infection and multiple organ dysfunction.
