D-二聚体联合彩色多普勒超声在全膝关节置换术后孤立性小腿深静脉血栓形成诊断中的应用

摘　　要：目的:探讨D-二聚体联合彩色多普勒超声在全膝关节置换术后孤立性小腿深静脉血栓形成(ICDVT)诊断中的应用价值。方法:回顾性分析2012年9月-2017年9月72例入院接受全膝关节置换术患者的临床资料,根据入院时间将患者分为两组,对照组34例和观察组38例,两组患者均在术后第1、3、5、7、14、21天接受D-二聚体检测,观察组患者除接受D-二聚体检测外,在术后第7、14、21天行肢体静脉彩色多普勒超声检查。结果:对照组34例患者中,11例患者D-二聚体水平≥10μg/mL,4例(11.8%)最终确诊ICDVT;观察组38例患者,19例患者D-二聚体水平≥10μg/mL,彩超提示ICDVT发病12例,13例(34.2%)确诊ICDVT。与对照组相比,观察组有较高的ICDVT检出率(P<0.05)。结论:与单纯D-二聚体检测相比,D-二聚体检测联合彩色多普勒超声检查的方式有助于全膝关节置换术后患者ICDVT的发现。Objective:To evaluate the value of D-dimer combined with color Doppler ultrasound in the diagnosis of isolated calf deep vein thrombosis(ICDVT)after total knee arthroplasty.Method:A total of 72 patients who admitted to hospital from Sep 2012 to Sep 2017 were divided into two groups according to admission time:the control group(n=34)and the observation group(n=38).All patients were examined with D-dimer on the 1st,3rd,5th,7th,14th and 21st day after operation,and the patients in the observation group received the color Doppler ultrasound examination of the veins in limbs on the 7th,14th and 21st day after operation besides the D-dimer test.Result:Among 34 patients in the control group,D-dimer level≥10μg/mL were found in 11 cases,4 cases(11.8%)were diagnosed with ICDVT.In the observation group,D-dimer level≥10μg/mL were found in 19 cases,color Doppler ultrasound indicated ICDVT in 12 cases,13 cases(34.2%)were diagnosed with ICDVT.Compared with the control group,the observation group had a higher detection rate of ICDVT(P<0.05).Conclusion:Compared with simple D-dimer detection,the combination of D-dimer detection and color Doppler ultrasound is helpful to the detection of ICDVT in patients with total knee arthroplasty.
