摘 要:目的探讨小儿哮喘治疗前后血清内皮素-1(ET-1)、一氧化氮(NO)、循环内皮细胞(CEC)的变化及其临床意义。方法选择2016年2月至2017年6月医院收治的68例哮喘患儿作为病例组,选择同期来医院体检的50例正常儿童作为对照组,检测病例组治疗前后患儿血清ET-1、NO、CEC水平,并与对照组比较,分析小儿哮喘ET-1、NO、CEC水平的变化及临床价值。结果病例组治疗前后血清ET-1、NO、CEC水平均高于对照组(P <0.05),病例组治疗后ET-1、NO、CEC水平低于同组治疗前(P <0.05);不同严重程度哮喘患儿ET-1、NO、CEC水平比较差异有统计学意义(P <0. 05),中重度哮喘患儿ET-1、NO、CEC高于轻度组(P<0.05),重度哮喘患儿ET-1、NO、CEC又高于中度组(P<0.05);有效组患儿血清ET-1、NO、CEC水平均低于无效组(P <0.05);ET-1、NO均与CEC呈正相关(P <0.05),ET-1与NO呈正相关(P <0.05)。结论哮喘患儿血清ET-1、NO、CEC水平存在明显异常增高特点,且上述因子水平的变化与哮喘病情严重程度及治疗效果有关,可作为哮喘病情评估及疗效预测的依据。Objective To investigate the changes of serum levels of endothelin-1 (ET-1),nitric oxide (NO)and circulating endothelial cells (CEC)in children with asthma before and after the treatment and their clinical significances. Methods Sixty-eight children with asthma who were admitted to the hospital from February 2016 to June 2017 were selected as the case group,while 50 healthy children who underwent physical examination in the hospital during the same period were selected as the control group.Serum levels of ET-1,NO and CEC in the both groups were detected before and after the treatment and then compared.The changes of ET-1,NO and CEC levels in children with asthma and their clinical values were analyzed. Results Serum levels of ET-1,NO and CEC in the case group were significantly higher than those in the control group before and after the treatment ( P <0.05).The levels in case group deceased significantly after the treatment (P<0.05).The levels in children with severe asthma were significantly higher than that in children with moderate asthma( P <0.05),and the levels in children with moderate and severe asthma were significantly higher than that in children with mild asthma ( P <0.05).The levels in the effective group were lower than those in the ineffective group ( P <0.05).Levels of ET-1 and NO were positively correlated with CEC ( P <0.05),while ET-1 was positively correlated with NO ( P <0.05). Conclusion Serum levels of ET-1,NO and CEC in children with asthma are significantly increased.The changes in the levels of these factors are related to the severity of asthma and the therapeutic effect,which can be used as a reference for assessing the condition of asthma and predicting the curative effect.
