益肺消纤方干预肺结核后继发纤维化的临床观察

摘　　要：目的:观察益肺消纤方联合西药治疗肺结核后继发纤维化的临床效果。方法:选取初治菌阳肺结核患者80例,按照随机数字表法分为对照组与观察组,各40例。对照组予以常规西药治疗,观察组在对照组基础上联合益肺消纤方治疗。比较两组血清纤维化指标(HA、LN)、痰菌阴转情况、肺部纤维化影像学变化、主要症状改善情况、临床疗效、不良反应。结果:观察组治疗6个月时的HA、LN水平均低于对照组(P<0.05);观察组治疗1、2个月时的痰菌阴转率均高于对照组(P<0.05);观察组治疗3个月时肺部纤维化影像学变化均优于对照组(P<0.05),治疗6个月时除炎症渗出外的其他指标均优于对照组(P<0.05);观察组主要症状改善情况及疗效均优于对照组(P<0.05);两组不良反应发生率比较差异无统计学意义(P>0.05)。结论:益肺消纤方联合西药对肺结核继发纤维化较单独使用西药治疗临床疗效显著,可有效减少纤维蛋白产生,且安全性高。Objective:To observe the clinical efficacy of Yifei Xiaoxian Decoction combined with western medicine in treatment of secondary fibrosis after tuberculosis.Method:A total of 80 newly treated patients with bacterium-positive pulmonary tuberculosis were selected.According to the random number table,they were divided into control group and observation group,40 cases in each group.The control group was treated with conventional western medicine,while observation group was treated with Yifei Xiaoxian Decoction on the basis of control group.The indexes of serum fibrosis(HA,LN),sputum negative conversion,imaging changes of pulmonary fibrosis,improvement of main symptoms,clinical efficacy and adverse reactions between two groups were compared.Result:The levels of HA and LN in observation group were lower than those of control group at 6 months of treatment(P<0.05).The sputum negative conversion rates in observation group were higher than those of control group at 1 and 2 months of treatment(P<0.05).The imaging changes of pulmonary fibrosis in observation group were better than those of control group at 3 months of treatment(P<0.05),the other indexes except inflammatory exudation were better than those of control group at 6 months of treatment(P<0.05).The improvement of main symptoms and curative effect in observation group were better than those of control group(P<0.05).The incidence of adverse reactions in two groups was compared,the difference was statistically significant(P >0.05).Conclusion:Yifei Xiaoxian Decoction combined with western medicine has a better clinical effect on secondary fibrosis of tuberculosis than western medicine alone,which can effectively reduce the production of fibrin and has high safety.
