CO_2激光显微手术与喉内镜下常规手术在声带息肉切除术中的疗效差异分析

摘　　要：目的探索激光显微手术与喉内镜下常规手术在声带息肉切除术中的疗效差异。方法方便收集该院2015年2月—2018年2月声带息肉患者62例。随机分为研究组和对照组,各31例。研究组采用CO_2激光显微手术,对照组采用喉内镜下常规手术。然后对两组患者进行电子喉镜、频闪喉镜检查。分析两组患者术前、术后1周、术后1个月和3个月的VHI主观评分、GRBAS、客观嗓音。结果术后1周研究组的嘶哑度是(2.1±0.6),术后1周对照组的嘶哑度是(1.6±0.8)。术后1周对照组的嘶哑度低于研究组,差异有统计学意义(t=2.784,P<0.05);术后1个月研究组的基频微扰百分比、基频扰动商、振幅微扰百分比分别是(3.2±0.6)%、(1.3±0.5)%、(3.3±0.4)%。术后1个月对照组的基频微扰百分比、基频扰动商、振幅微扰百分比分别是(3.2±0.6)%、(1.3±0.6)%、(3.6±0.9)%。研究组和对照组患者术后1个月的基频微扰百分比、基频扰动商、振幅微扰百分比差异无统计学意义(t=0.000,P>0.05;t=0.000,P>0.05;t=1.696,P>0.05)。结论 CO_2激光喉显微手术治疗声带息肉疗效优于喉内镜下常规手术。Objective To explore the difference in the curative effect of CO2 laser microsurgery and routine surgery under larynx endoscope in the polypectomy of vocal cord.Methods 62 cases of patients with polyp of vocal cord in our hospital from February 2015 to February 2018 were conveniently selected and randomly divided into two groups with 31 cases in each,the research group and the control group respectively sued the CO2 laser microsurgery and routine surgery under larynx endoscope,and then the two groups were for electronic laryngoscope and stroboscopic laryngoscopy,and the VHI subjective score,GRBAS and objective voice before surgery,in 1 week after surgery,in 1 month and 3 months after surgery were compared between the two groups.Results The hoarseness in the research group in 1 week after surgery was respectively(2.1±0.6)and(1.6±0.8),and the hoarseness in the control group in 1 week after surgery was lower than that in the study group,the different was statistically significant(t=2.784,P<0.05);and the percentage of fundamental frequency perturbation,fundamental disturbance quotient and amplitude perturbation percentage in 1 month after surgery in the research group and in the control group were respectively(3.2±0.6)%,(1.3±0.5)%,(3.3±0.4)%and(3.2±0.6)%,(1.3±0.6)%,(3.6±0.9)%,and the differences were not obvious(t=0.000,P >0.05;t=0.000,P >0.05;t=1.696,P >0.05).Conclusion The curative effect of CO2 laser microsurgery in treatment of polyp of vocal cord is better than that of routine surgery under larynx endoscope.
