内固定与外固定治疗不稳定性骨盆骨折的效果

摘　　要：目的探讨不稳定性骨盆骨折采用内固定与外固定治疗的效果。方法方便选取该院2013年2月—2018年2月期间内接收的50例不稳定性骨盆骨折患者,随机分为观察组与对照组,每组25例,分别接受内固定治疗、外固定治疗。对比治疗效果,观察手术基本情况以及术后不良反应发生情况,对比术后Majeed评分。结果观察组手术优良率96.00%较对照组80.00%来说更高(χ~2=12.121,P=0.000)。观察组术后总不良反应发生率12.00%较对照组32.00%来说显著更低(χ~2=11.655,P=0.001)。观察组手术时间、切口长度较对照组来说均更低(t=7.564、17.934,P=0.000、0.000)。观察组术后疼痛评分、工作能力评分、坐立评分、性生活质量评分以及Majeed总分较对照组来说均更高,比较差异有统计学意义(t=12.432、24.988、16.148、4.509、8.386、P<0.05)。观察组术中出血量较对照组稍高,但比较差异无统计学意义(t=4.487,P=0.143>0.05)。结论可对不稳定性骨盆骨折患者采用内固定治疗,效果更为确切,有效的改善了患者骨盆的功能,提高了关节活动范围,临床实用价值较为理想。Objective To study the effect of internal fixation and external fixation in treatment of unstable pelvic fractures.Methods 50 cases of patients with unstable pelvic fractures admitted and treated in our hospital from February 2013 to February 2018 were conveniently selected and randomly divided into two groups with 25 cases in each,respectively treated with internal fixation and external fixation,and the treatment effect was compared,and the operative basic condition and occurrence of postoperative adverse reactions were observed,and the postoperative Majeed score was compared.Results The operative excellent and good rate in the observation group was higher than that in the control group 96.00%vs 80.00%(χ^2=12.121,P=0.000),and the incidence rate of total adverse reactions in the observation group was obviously lower than that in the control group 12.00%vs 32.00%(χ^2=11.655,P=0.001),and the operative time,incision length in the observation group were lower than those in the control group(t=7.564,17.934,P=0.000,0.000),and the postoperative pain score,working ability score,standing score,sex life quality score and total Majeed score were higher than those in the control group,and the difference was statistically significant(t=12.432、24.998、16.148、4.509、8.386、P﹤0.05),and the intraoperative bleeding amount in the observation group was slightly higher than that in the control group,but and the difference was not statistically significant(t=4.487,P=0.143﹥0.05).Conclusion The effect of internal fixation in treatment of patients with unstable pelvic fractures is definite,which can effectively improve the pelvic function and improve the joint activity range,and the clinical practical value is ideal.
