摘 要:目的研究在气管切开机械通气患者中应用间断声门下吸引、持续声门下吸引的不同效果。方法方便选择该院2016年6月—2018年6月收治的106例气管切开机械通气患者进行研究,按照应用的声门下吸引法的不同进行分组,观察组53例、对照组53例,分别接受间断声门下吸引、持续声门下吸引,比较两组效果。结果观察组引流物细菌涂阳率、吸引管堵塞率、气囊上吸引量、机械通气时间与对照组差异无统计学意义(P>0.05),观察组呼吸机相关性肺炎发生率为24.53%,与对照组26.42%比较差异无统计学意义(χ~2=0.049 7,P=0.823 6),观察组每天刺激性呛咳发生次数少于对照组,吸引物隐血阳性率低于对照组(t=21.740 7、χ~2=9.112 3,P=0.000 0、0.002 5)。结论间断声门下吸引、持续声门下吸引用于气管切开机械通气患者效果接近,不过相比之下间断声门下吸引能够降低刺激性呛咳以及气道损伤发生率,更推广可行性。Objective To study the effect of aspiration methods at different glottises in patients with tracheotomy mechanical ventilation.Methods 106 cases of patients with tracheotomy mechanical ventilation admitted and treated in our hospital from June 2016 to June 2018 were convenient selected and divided into two groups with 53 cases in each according to different aspiration methods,and the patients respectively used the interrupted subglottic aspiration and continuous subglottic aspiration,and the effect was compared between the two groups.Results The differences in the rate of bacterial smear,suction tube blockage,suction on airbags,and duration of mechanical ventilation between the observation group and the control group were not statistically significant cant(P >0.05),and the difference in the incidence rate of ventilator associated pneumonia between the observation group and the control group was not obvious(24.53%vs 26.42%)(χ^2=0.049 7,P=0.823 6),and the occurrence frequency of irritant cough every day in the observation group was lower than that in the control group,and the positive rate of attraction occult blood was lower than that in the control group(t=21.740 7,χ^2=9.112 3,P=0.000 0,0.002 5).Conclusion The effect of interrupted subglottic aspiration and continuous subglottic aspiration in patients with tracheotomy mechanical ventilation is similar,and the former can reduce the incidence rates of irritant cough and airway damage better,and it is more suitable for feasibility.
