闭合复位交锁髓内钉治疗胫骨骨折的临床效果分析

摘　　要：目的探讨闭合复位交锁髓内钉治疗胫骨骨折的临床疗效与安全性。方法方便选取2012年1月—2016年12月该院80例胫骨骨折患者为研究对象,依据治疗方法不同分为实验组(闭合复位交锁髓内钉固定,40例)和对照组(锁定钢板内固定,40例),对比观察两组治疗指标,统计并发症,术后随访观察肢体功能恢复情况,评价疗效。结果实验组治疗优良率(92.50%)高于对照组(87.50%),但差异无统计学意义(χ~2=2.375,P>0.05);实验组术中出血量(64.8±15.1)m L,负重时间(9.3±1.0)周,骨折愈合时间(20.3±2.5)周,并发症发生率2.50%,各项指标均优于对照组,差异有统计学意义(t=7.531,3.015,3.420,χ~2=3.250;P<0.05)。结论闭合复位交锁髓内钉治疗胫骨骨折效果确切,患者并发症少,康复快,肢体功能恢复理想,值得临床推广使用。Objective To study the clinical curative effect and safety of closed reduction and interlocking intramedullary nail in treatment of tibial fracture.Methods 80 cases of patients with tibial fracture in our hospital from January 2012 to December 2016 were convenient selected as the research objects and divided into two groups with 40 cases in each according to different treatment methods,the experimental group used the closed reduction and interlocking intramedullary nailing,while the control group used the locked plate internal fixation,and the treatment indicators of the two groups were observed,and the complications were counted,and the recovery of limb function was followed up and observed,and the curative effect was evaluated.Results The treatment excellent and good rate in the experiment group was higher than that in the control group(92.50%vs 87.50%),the difference was no statistical significance(χ^2=2.375,P >0.05),and the intraoperative bleeding amount,loading time,fracture healing time and incidence rate of complications in the experimental group were respectively(64.8±15.1)mL,(9.3±1.0)w,(20.3±2.5)w and 2.50%,and various indicators were better than those in the control group,and the differences were statistically significant(t=7.531,3.015,3.420,χ^2=3.250;P﹤0.05).Conclusion The effect of closed reduction and interlocking intramedullary nail in treatment of tibial fracture is definite,with few complications and rapid recovery and ideal limb function recovery,and it is worth promotion and application.
