高龄产妇无痛分娩中应用导乐镇痛仪联合全程陪伴护理的效果及对分娩舒适度、产后心理状态的影响

摘　　要：目的观察导乐镇痛仪联合全程陪伴护理用于高龄产妇分娩效果及对分娩舒适度、产后心理状态影响。方法方便选取该院2016年6月—2018年6月收治的100例高龄产妇,随机分为观察组与对照组各50例,对照组给予常规产程护理,观察组给予导乐镇痛仪联合全程陪伴护理,比较两组分娩效果、分娩舒适度(疼痛PPI量表)、产后心理状态[爱丁堡产后抑郁量表(EPDS)]。结果干预后,观察组产程时间(382.42±55.67)min、产后出血量(158.34±26.72)mL、剖宫产率(8.00%)显著低于对照组的(498.51±64.38)min、(224.26±32.58)mL、24.00%(P<0.05),新生儿Apgar评分(9.34±0.32)分显著高于对照组(9.06±0.27)分(P<0.05);观察组各产程PPI评分(3.46±0.60)分、(2.98±0.52)分、(2.66±0.34)分显著低于对照组的(4.22±0.58)分、(3.64±0.76)分、(3.15±0.42)分(P<0.05),观察组产后EPDS评分(7.21±1.82)分显著低于对照组的(9.15±2.67)分(P<0.05),产后抑郁发生率4.00%,显著低于对照组的16.00%(χ~2=4.000,P<0.05)。结论导乐镇痛仪联合全程陪伴护理用于高龄产妇无痛分娩中,可改善分娩情况,促进分娩舒适,减少产后抑郁。Objective To observe the effect of Doula analgesia instrument combined with whole-course accompany nursing in the painless delivery of senile delivery women and effect on the delivery comfort degree and postpartum mental state.Methods 100 cases of senile delivery women admitted and treated in our hospital from June 2016 to June 2018 were convenient selected and randomly divided into two groups with 50 cases in each,the control group used the routine labor nursing,while the observation group used the Doula analgesia instrument combined with whole-course accompany nursing,and the delivery effect,delivery comfort degree(pain PPI scale),postpartum mental state[Edinburgh postnatal depression scale EPDS]were compared between the two groups.Results After intervention,the labor time,postoperative bleeding amount and cesarean section rate in the observation group were obviously lower than those in the control group[(382.42±55.67)min,(158.34±26.72)mL,8.00%,(498.51±64.38)min,(224.26±32.58)mL,24.00%(P﹤0.05),and the neonatal Apgar score was obviously higher than that in the control group[(9.34±0.32)points vs(9.06±0.27)points](P﹤0.05),and PPI scores of various labors in the observation group were obviously lower than those in the control group[(3.46±0.60)points,(2.98±0.52)points,(2.66±0.34)points vs(4.22±0.58)points,(3.64±0.76)points,(3.15±0.42)points](P﹤0.05),and the postpartum EPDS score in the observation group was obviously lower than that in the control group[(7.21±1.82)points vs(9.15±2.67)points](P﹤0.05),and the incidence rate of postpartum depression was obviously lower than that in the control group(4.00%vs 16.00%,χ^2=4.000,P﹤0.05).Conclusion The application of Doula analgesia instrument combined with whole-course accompany nursing can improve the delivery condition,promote the comfort delivery and reduce the postpartum depression.
