摘　　要：目的探讨Ilizarov技术治疗儿童双下肢不等长的治疗效果。方法回顾性分析2011年1月至2016年12月该院收治住院的采用Ilizarov技术进行骨延长治疗双下肢不等长31例患儿的临床资料。结果观察31例患儿术前及术后肢体长度均不服从正态分布,随访6～60个月,平均25个月,肢体短缩长度为2.2～16.0cm,术前双下肢长度差距4.4cm(3.5cm,6.5cm),术后双下肢长度差距0.5cm(0.3cm,1.2cm),骨延长1.4～6.9cm,平均3.9cm。手术前后肢体长度比较,差异有统计学意义(P<0.05)。术后优21例(67.74%),良7例(22.58%),中2例(6.45%),差1例(3.23%)。男性与女性及股骨与胫腓骨治疗效果比较,差异均无统计学意义(P>0.05)。创伤性及非创伤性治疗效果比较,差异有统计学意义(P <0.05)。结论 Ilizarov技术能在一定程度上矫正下肢的成角畸形,对治疗创伤性双下肢不等长治疗效果较好,且具有方法简单、手术创伤小、不需要植骨、并发症较少等优点。Objective To evaluate the treatment effect of limb-length discrepancies in children.Methods The clinical data of 31 cases limb-length discrepancies in children who were treated by the Ilizarov fixator for bone lengthening treatment in the hospital from January 2011 to December 2016 were analyzed retrospectively.Results The length of the limb in preoperative and postoperative were disobeying normal distribution.The patients were followed up 6-60 months,the average was 25 months.The length of the limb shortening was 2.2-16.0 cm,the difference of the limb-length in preoperative was 4.4 cm(3.5 cm,6.5 cm),the difference of the limb-length in postoperative was 0.5 cm(0.3 cm,1.2 cm),the length of the limb lengthening was 1.4-6.9 cm,the average was 3.9 cm.There was significant statistical difference in the length of limb in preoperative and postoperative(P<0.05).The functional results were found to be excellent in 21 cases(67.74%),good in 7 cases(22.58%),ok in 2 cases(6.45%)and fair in 1 case(3.23%).There were all no significant statistical difference of therapeutic effect between male and female,femur and tibia(P >0.05).There was significant statistical difference of therapeutic effect between traumatic group and non-traumatic group(P<0.05).Conclusion The Ilizarov fixator is an effective method for limb-length discrepancies in children,and to some extent can corrective partial angular deformity.It has superiority of simple operation,little trauma,don′t need bone grafting and fewer complication.
