摘 要:目的探讨新诊断弥漫性毒性甲状腺肿(GD)的女性患者中25羟维生素D[25 (OH) D]水平及其与GD的关系。方法选取个旧地区74例新诊断GD女性患者作为患者组,53例健康体检女性作为健康组。采用liason化学发光法测定2组血清25 (OH) D及甲状腺功能[总三碘甲状腺原氨酸(TT3)、游离三碘甲状腺原氨酸(FT3)、总甲状腺素(TT4)、游离甲状腺素(FT4)、促甲状腺激素(TSH)]、促甲状腺素受体抗体(TRAB)指标水平并进行比较分析,应用多元线性逐步回归分析25 (OH) D水平的影响因素。结果 (1)个旧地区健康对照组及GD组人群血25 (OH) D水平分别为(19.91±6.36) ng/mL和(20.60±9.66) ng/m L,两组间无统计学差异(P>0.05);(2) GD组患者25 (OH) D充足、不足及缺乏比例分别为5.66%、43.40%、50.94%,25 (OH) D充足、不足、缺乏的比例2组间比较无统计学差异(P> 0.05);(3)多因素线性回归分析提示25(OH) D水平与PTH负相关(P<0.001),与TT3水平正相关(P=0.0147)。结论 (1)个旧地区新诊断女性GD患者存在明显维生素D缺乏;(2) GD患者25 (OH) D水平及缺乏的比例与健康对照组比较无差异;(3) GD患者25 (OH) D水平与PTH负相关,与TT3水平正相关。Objective To investigate the 25-hydroxy vitamin D[25(OH)D]level in women with newly diagnosed Graves disease and the relationship between 25(OH)D and GD.Methods 74 cases of women with newly diagnosed GD and 53 cases of healthy women(control group)in Gejiu were selected.The levels of serum 25(OH)D,thyroid functions and thyrotropin receptor antibodies were detected by chemiluminescence method of liason.Clinical data were analyzed by SPSS 19.0 statistical soft packet.Tests,such as single factor analysis of variance test,non-parametric test and Chi-square test,were used to make comparisons between the two groups,Multiple regression analysis was used to analyze 25(OH)D related factors.Results Mean serum 25(OH)D in the control group and GD group in Gejiu were(19.91±6.36)ng/mL and(20.60±9.66)ng/mL respectively,and there was no difference between the two groups(P >0.05).(2)The prevalence of 25(OH)D adequate/deficiency/insufficiency in GD group were 15.66%、43.40%、50.94%,respectively.The proportion of 25(OH)D adequate,deficiency,insufficiency showed no difference between the two groups(P >0.05).(3)The multiple linear regression analysis showed that TT3 was positively related with 25(OH)D and PTH was negatively related with 25(OH)D in the GD group(P<0.001,P=0.0147 respectively).Conclusion 25(OH)D defficiency/insufficiency were widespread in women with newly diagnosed Graves disease in Gejiu area.Not only the level of 25(OH)D,but also the proportion of25(OH)D insufficiency had no difference between the two groups.TT3 was positively related with 25(OH)D and PTH was negatively related with 25(OH)D in the GD group.
