摘　　要：借助上海市建立徐汇区-中山医院医联体的契机,上海徐汇云医院创建了"互联网+分级诊疗"的医疗服务模式。云医院提供以"视频看医生"为核心的互联网医疗服务,建立了全-专云平台,实现了云医院与社区卫生服务中心的医疗信息系统的互联互通。社区卫生服务中心的全科医生登录全-专云平台后,可向上级医院的专科专家进行实时咨询或请求其会诊,也可为患者预约到上级医院就诊、检查或住院等。云医院亦向医联体外的医疗或医疗相关机构提供医疗健康服务,如养老院、社会药店等,以协助解决日常的医疗问题。云医院今后将整合终端智能医疗健康监测技术与服务,大力发展可穿戴设备和机器人系统的应用,开创慢性病管理新模式,建立区域性的人工智能辅助诊断中心,促进区域内医疗资源的大融合,为分级诊疗的实施提供物质基础和保证。A mode of "internet+ hospital hierarchical diagnosis and treatment" was created in Shanghai Xuhui Cloud Hospital (SXCH) by taking advantage of the establishment of a medical union of Xuhui District-Zhongshan Hospital. The internet medical services centered on "seeing a doctor by video" could be provided in SXCH by establishing a general-specialist doctor cloud-work-platform, by which the interconnection of medical information systems between SXCH and community health service centers could be achieved. General practitioners can consult a specialist in a higher level hospital for real-time consultation or request a consultation and can also make an appointments for a patient to go to a higher level hospital for treatment, examination or hospitalization and so on. SXCH can also extend their medical and health services to medical or not-medical institutions outside the medical union, such as nursing homes, social pharmacies, to help solve daily medical problems. SXCH will integrate terminal intelligent medical health monitoring technology and services, develop wearable equipment and intelligent robot systems, innovate a mode of chronic disease management and establish regional artificial intelligence assistant diagnosis center in the future. By these initiatives, SXCH will also promote the integration of medical resources in the region and provide material basis and guarantee for the implementation of hospital hierarchical diagnosis and treatment.
