探讨在直接经皮冠脉介入治疗中半剂量欣维宁对急性ST段抬高型心肌梗死男性患者的疗效

摘　　要：目的:探讨半剂量血小板膜蛋白(GP)Ⅱb/Ⅲa受体拮抗剂在男性急性心肌梗死患者直接PCI治疗的疗效及安全性。方法:连续入选2015年12月至2017年12月在我院心内科就诊并行直接PCI的急性ST段抬高型心肌梗死患者200名,分为半剂量组和常规剂量组,观察出血等并发症。结果:半剂量组104例,常规剂量组96例,2组间基础临床资料无统计学差异(P>0.05)。出血的并发症半剂量组与常规剂量组相比较,为4.8%:13.5%,P<0.05,统计学上有差异;术后支架内血栓形成的并发症相比较,P>0.05,无统计学意义;血小板减少在常规剂量组术前及术后24小时比较,P<0.05,统计学上有差异。结论:半剂量欣维宁在男性急性ST段抬高型心肌梗死患者行直接PCI治疗中使用是安全有效的,我们建议在高出血风险患者使用半量欣维宁抗血栓治疗。Objective:To evaluate the efficacy and safety of use of platelet membrane glycoprotein(GP)Ⅱb/Ⅲa inhibitor tirofiban for primary percutaneous coronary interventional(PCI)in the male patients with acute ST segment elevation myocardial infarction(STEMI).Methods:A total of 200 patients who suffered from STEMI and underwent PCI were selected from Dec 2015 to Dec 2017.The patients were randomly assigned to two groups:half-dose tirofiban group and standard-dose tirofiban group,we observed their complications,the bleeding,stent thrombosis.Results:Total 200 patients accomplished in the study,there were 104 patients in the half-dose tirofiban group,96 patients in the standard-dose tirofiban group.The incidence of bleeding complications were statistical significance in both gloups,P<0.05,the stent thrombosis in both gloups were no statistical significance,P>0.05.Conclusions:It is the efficacy and safety to use GPⅡb/Ⅲa inhibitor tirofiban for PCI in male patients with STEMI,half-does tirofiban can offer the same level of efficacy as standard-dose,with less associated bleeding.
