系统性红斑狼疮患者血清LncRNA MIR155HG的表达及临床意义

摘　　要：目的探讨LncRNA MIR155HG在系统性红斑狼疮患者临床诊治中的价值。方法采用反转录PCR法(RT-PCR)检测80例2016年1月至2017年12月来我院门诊和住院就诊的系统性红斑狼疮(SLE)患者及40例同期来我院健康体检的对照者的血清LncRNA MIR155HG的表达,分析其表达与临床组患者血清LncRNA MIR155HG的表达水平显著增高,SLE活动期患者组高于SLE稳定期患者,差异均有统计学意义(P <0.001)。LncRNA MIR155HG的表达与系统性红斑狼疮疾病活动度(SLEDAI)呈正相关,差异均有统计学意义(P <0.001)。SLE合并狼疮肾炎患者血清LncRNA MIR155HG的表达明显高于无肾炎组,差异均有统计学意义(P <0.05)。血清LncRNAMIR155HG作为诊断SLE患者血清标志物的ROC灵敏度和特异性分别为81.3%(阈值15%)和90.2%(阈值30%)。血清LncRNA MIR155HG作为诊断是否合并狼疮肾炎的ROC曲线下面积为0.915(95%CI:0.870～0.960;P <0.001),灵敏度和特异性分别为82.5%(阈值20%)和90.8%(阈值30%)。结论血清LncRNA MIR155HG在SLE患者中高表达,与SLE患者的活动度相关,LncRNA MIR155HG有可能成为一个新的SLE疾病活动度评估及预后判断的分子标志物。Objective To explore the value of LncRNA MIR155HG in the diagnosis and treatment of pa tients with systemic lupus erythematosus.Methods Eighty patients with systemic lupus erythematosus(SLE)and 40 cases of healthy controls from January 2016 to December 2017 were involved in the study.The expression of serum LncRNA MIR155HG in the 2 groups was detected by reverse transcriptional PCR(RT-PCR),and its association with the clinicopathological features was analyzed.Results Compared with that in the control group,the expression level of serum LncRNA MIR155HG in the SLE group was significantly increased.The expression was higher in pa tients with active SLE than that in those with stable SLE(P<0.001).The expression of LncRNA MIR155HG was positively correlated with disease activity in systemic lupus erythematosus(SLEDAI)and the difference was statisti cally significant(P<0.001).The expression of LncRNA MIR155HG in patients with lupus nephritis combined with SLE was significantly higher than that in patients without nephritis(P<0.05).The area under the receiver operat ing characteristic curves of serum LncRNA MIR155HG as a serum marker for the diagnosis of SLE was 0.720(95%CI,0.601~0.840;P<0.001),and the sensitivity and specificity were 81.3%(threshold 15%)and 90.2%(thresh old 30%),respectively.The area under the ROC curve of serum LncRNA MIR155HG as a diagnosis of lupus ne phritis was 0.915(95%CI,0.870~0.960,P<0.001),and its sensitivity and specificity were 82.5%(threshold 20%)and 90.8%(threshold 30%),respectively.Conclusion Serum LncRNA MIR155HG is highly expressed in SLE patients and is related to the activity of SLE patients.LncRNA MIR155HG may be a new molecular marker for evaluating the activity of SLE disease and predicting the prognosis.
