摘  要:目的观察兔面神经周围支损伤后,电针对上皮钙黏素(E-cad)和钙调素(CaM)在中枢面神经元表达的影响及两者的相关性,探讨电针促进周围面神经再生可能的作用方式和途径。方法新西兰大白兔52只,设立正常组(4只,不做任何处理)、手术组(24只,建立右侧面神经上颊支压榨损伤模型)和电针组(24只,建模后穴位电针治疗)。手术组和电针组于术后1、4、7、14、21、28 d取材,采用免疫组化法和实时定量聚合酶链反应(qRT-PCR)检测面神经元中E-cad和CaM的表达,分析其相关性。结果电针组兔周围性面瘫症状较手术组恢复快且完全。免疫组化显示,除术后14 d(E-cad)和术后21 d(CaM),电针组E-Cad和CaM阳性细胞数均明显多于手术组(P <0.001,P <0.05)。电针组面神经元E-cad和CaM的相对表达量均较手术组及正常组高(P <0.05)。2组E-cad和CaM的表达有关联性。结论电针能有效治疗周围性面瘫,并可能通过促进面神经元中E-cad和CaM的高表达,使表达高峰提前并延长,发挥面神经元保护的作用,从而促进周围面神经再生。Objective To investigate the expression of E-cadherin(E-cad)and Calmodulin(CaM)in facial motoneurons and the correlation between them in New zealand rabbit model of peripheral facial nerve crush injury,as well as to illustrate the effection of electroacupuncture on promoting facial nerve regeneration and to reveal the underlying mechanism.Methods Fifty-two rabbits were randomly divided into the normal group(n=4),the surgery group(n=24,underwent facial nerve crush injury only)and the electroacupuncture group(n=24,received electroacupuncture after surgery).Tissue samples were collected at 1,4,7,14,21 and 28 days post-surgery.Immunohistochemical techniques and real-time quantitative polymerase chain reaction(qRT-PCR)were performed to detect the expression of E-cad and CaM in facial motoneurons.Then the correlation between them was analyzed.Results After the crush injury in peripheral facial nerve,symptoms of facial paralysis in the electroacu-puncture group were observed earlier than those in the surgery group,and could be completely recovered.Except for 14 days post-operation(E-cad)and 21 days post-operation(CaM),the expression of E-Cad and CaM in two groups had significant difference at other time points(P﹤0.001,P﹤0.05,respectively).In the electroacupuncture group,the number of E-Cad and CaM positive cells were significantly more than those in the surgery group.The relative expression of E-cad and CaM mRNA in the electroacupuncture group was higher than that in the surgery group and in the normal group(P﹤0.05,respectively).There was a correlation between the expression of E-cad and CaM in the two groups.Conclusions Electroacupuncture could effectively alleviate peripheral facial paralysis and promote the peripheral facial nerve regeneration,possibly through upregulating the expression of E-cad and CaM in facial motoneurons.
