摘　　要：目的探讨LncRNA KCNQ1OT1(KCNQ1OT1)在Ⅱ型糖尿病(T2D)、糖尿病肾病(DN)患者血清中的表达及意义。方法 qRT-PCR法检测KCNQ1OT1在77例T2D患者,60例DN患者,60例健康对照组中的表达,分析其临床意义。结果 KCNQ1OT1在T2D及DN患者中的表达较健康对照组明显升高(F=19.790,P <0.001)。KCNQ1OT1在合并大量蛋白尿的DN患者中的表达明显高于合并微量蛋白尿患者(t=13.620,P <0.001)。KCNQ1OT1的表达与蛋白尿的水平呈正相关关系(r=0.690,P <0.001),与肾小球滤过率呈负相关关系(r=-0.780,P <0.001)。在T2D与DN患者中KCNQ1OT1作为糖尿病肾病患者血清标志物的潜能的ROC曲线下面积(AUC)为0.861(95%CI:0.786～0.935,P <0.001),在DN患者与健康患者中KCNQ1OT1作为糖尿病肾病的AUC为0.914(95%CI:0.828～0.980,P <0.001)。结论 KCNQ1OT1的高表达与T2D患者DN的发展有关,有望成为预测DN患者预后的生物标志物。Objective To explore the expression and significance of circulating LncRNA KCNQ1OT1 in typeⅡdiabetes mellitus(T2D)and diabetic nephropathy(DN)patients.Methods The expression of KCNQ1OT1 in 77 patients with T2D,60 patients with DN and 60 healthy controls were enrolled and were detected by qRTPCR,and its clinical significance was analyzed.Results The expression of KCNQ1OT1 in T2D and DN patients was significantly higher than that in healthy controls(F=19.790,P﹤0.001).The expression of KCNQ1OT1 in DN patients with massive proteinuria was significantly higher than that in DN patients with microalbuminuria(t=13.620,P﹤0.001).The expression of KCNQ1OT1 was positively correlated with proteinuria(r=0.690,P﹤0.001)and negatively correlated with glomerular filtration rate(r=-0.780,P﹤0.001).The area under the ROC curve(AUC)of KCNQ1OT1 as a serum marker in diabetic nephropath in patients with T2D and DN was 0.861(95%CI:0.786~0.935,P﹤0.001),and in patients with DN and healthy controls was 0.914(95%CI:0.828~0.980,P﹤0.001).Conclusion The high expression of KCNQ1OT1 is associated with the development of DN in patients with T2D,and may be a biomarker for predicting the prognosis of DN.
