不明原因复发性流产患者外周血Treg细胞及细胞因子TGF-β、IL-10的表达及临床意义

摘　　要：目的研究不明原因复发性流产(URSA)患者外周血中Treg细胞及细胞因子TGF-β及IL-10的变化。方法本研究纳入2013年9月至2015年1月在中山大学孙逸仙纪念医院就诊的有URSA病史的稽留流产患者、有URSA病史未孕患者各50例;以正常早孕者及正常未孕者各25例作为正常对照。检测外周血Treg细胞百分比;Foxp3 mRNA表达;血清中细胞因子TGF-β及IL-10的表达。结果正常早孕者Treg细胞百分比较正常未孕者增高;URSA稽留流产者Treg细胞百分比较URSA未孕者增高;URSA稽留流产患者Treg细胞百分比较正常早孕者明显下降;正常早孕者Foxp3 mRNA水平较正常未孕者增高;URSA稽留流产者Foxp3 mRNA水平较URSA未孕者增高;URSA稽留流产患者Foxp3 mRNA水平较正常早孕者下降;URSA稽留流产患者TGF-β及IL-10水平均较正常早孕者下降,差异均有统计学意义。结论外周血Treg细胞数量及功能下降可能与URSA的发生密切相关。Objective To study the changes of Treg cells and cytokines TGF-beta and IL-10 in peripheral blood of patients with unexplained recurrent abortion(URSA).Methods All subjects were recruited from the department of Obstetrics and Gynecology of Sun Yat-sen Memorial Hospital in China between September 2013 and January 2015.URSA non-pregnant women 50 cases,URSA missed abortion patients 50 cases,normal non-pregnant women 25 cases and normal early pregnant women 25 cases.The proportions of Treg cells,Foxp3 mRNA and serum level of TGF-βand IL-10 were detected.Results The proportions of Treg cells were significantly higher in normal pregnant women than in non-pregnant women and also higher in URSA missed abortion patients than in URSA non-pregnant women.The proportions of Treg cells were significantly lower in URSA missed abortion patients than in normal controls.The levels of Foxp3 mRNA were significantly higher in normal pregnant women than in non-pregnant women and also higher in URSA missed abortion patients than in URSA non-pregnant women.The levels of Foxp3 mRNA were significantly lower in URSA missed abortion patients than in normal controls.Cytokines TGF-βand IL-10 were also lower in URSA patients than in normal controls.Conclusion The decrease in the number and function of Treg cells in peripheral blood may be closely related to the occurrence of URSA.
