摘　　要：目的观察纳络酮治疗重症手足口病(HFMD)前后血清、脑脊液中β-内啡肽(β-EP)、免疫球蛋白、白细胞亚群的变化,探讨重症手足口病的神经免疫学指标变化、意义以及治疗对策。方法按照临床诊断标准,将60例HFMD重症患儿随机分成两组,每组30例,分别予以常规治疗和常规加纳络酮治疗,治疗前后均采集血清进行β-EP、免疫球蛋白、白细胞亚群检测,同时采集脑脊液进行β-EP检测。结果纳络酮治疗前后重症患儿血清、脑脊液β-EP呈显著下降;IgG、IgA、IgM、CD4/CD8显著升高,CD8显著下降,纳洛酮治疗临床效果明显。结论β-EP是重症HFMD紊乱参与重症HFMD进展;纳洛酮可作为重症患儿有效治疗对策之一。Objectives To investigate the significance of neuroimmulogical indicators changes and treatment countermeasures of severe hand foot and mouth disease(HFMD)by observing the changes ofβ-Endorphin(β-EP),immunoglobulin and leukocyte subpopulations in serum and cerebrospinal fluid before and after the treatment of naloxone.Methods 60 children with HFMD in critical condition were chosen according to the clinical diagnosis criteria and divided into two groups.Every group had 30 children,one group was treated conventionally and another group was taken conventional treatment and naloxone.The serum was collected to detect theβ-EP,immunoglobulin and leukocyte subpopulations as well as the cerebrospinal fluid was taken to detectβ-EP before and after the treatment.Resultsβ-EP in serum and cerebrospinal fluid were significantly decreased while IgG,IgA,IgM,CD4/CD8 were significantly increased and CD8 was significantly decreased among 60 severe HFMD patients after the treatment of Naloxone.The treatment was significantly effective.Conclusionsβ-EP is an important medium in the development of severe HFMD.The adjustment disorder of Nerve-immune network takes part in the development of severe HFMD.Naloxone is one of the effective countermeasures to treat severe HFMD.
