血清TK1、HE4、CA125联合检测在卵巢恶性上皮性肿瘤诊断中的价值

摘　　要：目的探讨血清肿瘤标记物联合检测对卵巢肿瘤的诊断价值。方法选取138例卵巢单纯囊肿(对照组)、115例卵巢上皮性良性肿瘤(良性组)、122例卵巢上皮性癌(恶性肿瘤组),术前分别检测血清中胸苷激酶1(TK1)、人附睾分泌蛋白4(HE4)、糖类抗原蛋白125(CA125)、糖类抗原蛋白199(CA199)水平,采用ROC曲线分析其单个及联合检测对上皮性卵巢癌患者诊断价值。结果 3组患者间血清CA125、CA199、HE4、TK1水平差异均有统计学意义(P均<0.05);与对照组比较,良性肿瘤组和恶性肿瘤组CA125、CA199、HE4、TK1表达均增高(P均<0.05);与良性肿瘤组比较,恶性肿瘤组中CA125、CA199、HE4、TK1表达增高(P均<0.05)。CA125、CA199、HE4、TK1的AUC值分别为0.689、0.590、0.746、0.750;CA125、HE4、TK1三者联合诊断AUC值为0.899,均高于单独及其他联合分组。结论血清HE4、TK1、CA125三者联合检测能提高卵巢癌患者的诊断率。Objective To explore the diagnostic value of combined detection of tumor markers in ovarian malignant tumors.Methods Total 138 cases of ovarian cyst(control group),115 cases of ovarian benign tumors(benign group),122 cases of ovarian epithelial carcinoma group(malignant tumor)were selected.The expression of TK1,HE4,CA125 and CA199 was detected before operation in different groups.The diagnostic value of single and combined detection of TK1/HE4/CA125/CA199 was analyzed by ROC curve.Results The levels of serum CA125,CA199,HE4 and TK1 were significantly different among the three groups(P<0.05).Compared with the control group,the expressions of CA125,CA199,HE4 and TK1 in benign tumors and malignant tumors were all increased(P<0.05).Compared with benign tumors,the expressions of CA125,CA199,HE4 and TK1 in malignant tumors were increased(P<0.05).The AUC values of CA125,CA199,HE4 and TK1 were 0.689,0.590,0.746 and 0.750,respectively.The combined diagnostic AUC values of CA125,HE4 and TK1 were 0.899,which were higher than those in other combined groups.Conclusion Combined detection of serum HE4/TK1/CA125 can significantly improve the early diagnostic rate on ovarian cancer.
