摘　　要：目的探讨无栓病房建立对下肢骨折患者术后患肢肿胀及预后的影响。方法采用简单随机抽样法抽取2016年3月至2017年2月的300例下肢骨折患者设为对照组,按照下肢骨折围手术护理常规实施护理; 2017年3月至2018年5月的300例患者设为观察组,在对照组常规护理基础上建立无栓病房及管理,观察比较两组患者术后第7天患肢肿胀的疗效及入院时、术后第1天、第3天、第7天患肢周径和疼痛状况。结果术后第7天,观察组患肢肿胀疗效优于对照组,术后第1天、第3天、第7天患肢周径和数字化疼痛评估法(NRS)评分观察组均低于对照组,差异有统计学意义(P<0.01)。结论无栓病房建立能减轻下肢骨折患者术后患肢肿胀和疼痛的程度,提高患肢肿胀的疗效。Objective To investigate the effect of establishment of thrombosis-free ward on the swelling and prognosis of patients with lower limb fracture after operation. Methods 300 cases of lower limb fracture patients from March 2016 to February 2017 selected by simple random sampling method were included in control group,and the nursing was carried out according to the perioperative nursing routine of lower limb fracture in this group.A total of 300 patients from March 2017 to May 2018 were included in observation group,and thrombosis-free ward was established for this group on the basis of routine nursing care in the control group.And then,the curative effects of limb swelling on the 7th day after operation as well as the circumference of affected limb and pain status of the affected limbs on admission,on the 1st,3rd and 7th day after operation were observed and compared between the two groups. Results On the 7th day after operation,the curative effect of limb swelling in the observation group was better than that in the control group,and on the 1st,3rd and 7th day after operation,the circumference of affected limbs and NRS score in the observation group were lower than those in the control group,difference was statistically significant( P <0.01). Conclusion The establishment of thrombosis-free ward can reduce the degree of postoperative swelling and pain of the affected limb in patients with lower limb fracture and promote the curative effect of the affected limb swelling.
