改良椎间孔椎间融合术联合椎弓根螺钉内固定治疗Ⅰ～Ⅱ度腰椎滑脱症  

Modified transforaminal lumbar interbody fusion coupled with pedicle screw fixation for the treatment of grade Ⅰ and Ⅱ lumbar spondylolisthesis

摘　　要：目的探讨经改良椎间孔椎间融合术(TLIF)椎弓根螺钉内固定治疗Ⅰ～Ⅱ度腰椎滑脱的临床疗效。方法采用经改良TLIF椎弓根螺钉内固定治疗Ⅰ～Ⅱ度腰椎滑脱患者25例。全部患者来自于2013年5月至2015年4月于海军安庆医院收治的部分患者。结果术后日本骨科协会评估治疗(JOA)改善率平均80%,所有病例的腰椎滑脱均得到完全或近解剖复位,无神经根硬膜囊损伤、无椎间隙感染,所有椎间骨性融合良好,术后2例出现对侧神经根症状,保守治疗1月后恢复。结论经改良TLIF椎弓根螺钉内固定是治疗Ⅰ～Ⅱ度腰椎滑脱安全、有效的方法。Objective To investigate the clinical efficacy of modified transforaminal lumbar interbody fusion(TLIF)coupled with pedicle screw fixation for the treatment of gradeⅠ and Ⅱ lumbar spondylolisthesis.Methods Twenty-five cases of gradeⅠ andⅡ lumbar spondylolisthesis were treated with modified TLIF coupled with pedicle screw fixation.Results After surgery,the rate of JOA improvement was on the average 80%,and all the cases of lumbar spondylolisthesis completely or nearly restored anatomically,with no damage to nerve roots,no infection of dural sac intervertebral space and good intervertebral interbody fusion.After surgery,2 cases displayed symptoms of contralateral radicular pain,but both patients recovered after a month of conservative treatment.Conclusion The modified TLIF coupled with pedicle screw fixation is a safe and effective method for the treatment of spondylolisthesis.
