院内移动ICU在严重失血性休克患者抢救中的应用

摘　　要：目的探讨院内移动ICU在严重失血性休克患者抢救中的应用效果。方法研究对象为2015年1月到2017年1月我院收治的68例严重失血性休克患者,根据患者院内移动方法分为移动ICU组(n=34)和常规组(n=34),给予移动ICU组患者采取移动ICU护理,给予常规组患者采取常规护理。对比两组患者的抢救效果。结果移动ICU组患者在住院期间及转运过程中均无死亡病例。常规组中4例患者在转运过程中死亡,其中2例患者因消化性溃疡出血,另2例患者因腹部损伤合并颅脑外伤严重,在转运过程中突发呼吸、心跳骤停抢救不及时死亡。移动ICU组患者在转运中死亡率以及住院期间总死亡率与常规组相比明显较低,P<0. 05,差异具备统计学意义。移动ICU组患者家属满意度评分为(27. 6±1. 8)分,明显高于常规组为(23. 1±2. 4)分,P<0. 05,差异具备统计学意义。结论院内移动ICU能有效改善患者院内移动安全性,提高患者抢救效果,改善护患关系,可在严重失血性休克患者抢救中推广运用。Objective To explore the application effect of in-hospital mobile ICU mode in rescuing patients with severe hemorrhagic shock.Methods 68 hospitalized cases with severe hemorrhagic shock during the period from Jan.,2015 to Jan.,2017 were selected and were divided,according to the mobile ways,into 2 groups:mobile ICU group and routine group,34 in each;routine nursing was applied to cases in routine group while ICU nursing was applied to cases in mobile ICU group;a comparative study was made to the rescuing outcome between the 2 groups.Results No death occurred in mobile ICU group during hospitalization or in in-hospital mobile while 4 cases died in routine group:2 due to peptic ulcer hemorrhage and the other 2 with severe abdominal injury plus craniocerebral trauma due to sudden attack of respiratory and cardiac arrest in mobile;the total mortality in mobile and during hospitalization in mobile ICU group was obviously lower with statistical significance than that in routine group(P<0.05)while the satisfaction scores of patients'families in mobile ICU group were much higher than those in routine group[(27.6±1.8)vs(23.1±2.4)](P<0.05).Conclusions In-hospital mobile ICU mode can promote the safety in in-hospital mobile of patients,raise the rescuing effect and improve the nursing-patient relationship;it is worthy of wider application in rescuing patients with severe hemorrhagic shock.
