摘 要:目的分析海南地区2型糖尿病老年患者并发冠状动脉狭窄风险与乙醛脱氢酶2(ALDH2)基因多态性的关系。方法选择海南本地2型糖尿病老年患者300例,根据冠状动脉造影结果分为冠心病组(单支或多支冠状动脉狭窄程度≥30%) 159例和非冠心病组141例。采集患者空腹外周静脉血10 m L,采用RT-PCR法测定两组ALDH2基因型,采用Logistic回归分析ALDH2基因多态性与冠状动脉狭窄的相关性。结果患者ALDH2基因型有3种,即GG型、AG型、AA型。冠心病组和非冠心病组AA基因型频率、AG基因型频率低于GG基因型频率,A等位基因频率低于G等位基因频率。冠心病组GG基因型频率低于非冠心病组,AA基因型频率高于非冠心病组;G等位基因频率低于非冠心病组,A等位基因频率高于非冠心病组(P均<0. 05)。不同性别患者AA基因型频率、AG基因型频率低于GG基因型频率,A等位基因频率低于G等位基因频率(P <0. 05)。不同性别ALDH2基因型频率、等位基因频率分布差异无统计学意义(P均> 0. 05)。≤70岁者与> 70岁者ALDH2基因型频率、等位基因频率分布差异无统计学意义(P> 0. 05)。≤70岁者与> 70岁者的AA基因型频率、AG基因型频率均低于GG基因型频率,A等位基因频率低于G等位基因频率(P均<0. 05)。Logistic分析显示,A等位基因是影响冠状动脉狭窄发病的危险因素(OR=2. 426,95%CI为1. 312~4. 526,P=0. 005)。结论海南地区2型糖尿病老年患者并发冠状动脉狭窄风险与ALDH2基因多态性有关,ALDH2的A等位基因增加了2型糖尿病老年患者发生冠状动脉狭窄的危险。Objective To investigate the association between the risk of coronary artery stenosis and the polymorphism of aldehyde dehydrogenase 2(ALDH2)gene in elderly patients with type 2 diabetes mellitus in Hainan.Methods According to the results of coronary angiography,300 patients with type 2 diabetes mellitus in Hainan were divided into the coronary heart disease group(stenosis of single or multiple coronary arteries≥30%,n=159,CHD group)and non-coronary heart disease group(n=141,non-CHD group).The fasting peripheral venous blood(10 mL)was collected from patients,and we used RT-PCR to detect the ALDH2 genotypes,and used Logistic regression to analyze the association between ALDH2 gene polymorphism and coronary artery stenosis.Results There are three ALDH2 genotypes,namely GG,AG,and AA.The frequencies of AA genotype and AG genotype in the CHD group and non-CHD group were lower than that of GG genotype,and the frequency of A allele was lower than the frequency of G allele.The frequency of GG genotype in the CHD group was lower than that in the non-CHD group,the frequency of AA genotype was higher than that in the non-CHD group,the frequency of G allele was lower than that in the non-CHD group,and the frequency of A allele was higher the frequency that in non-CHD group(all P<0.05).The frequencies of AA genotype and AG genotype were lower than the frequency of GG genotype,and the frequency of A allele was lower than the frequency of G allele(all P<0.05).There was no significant difference in ALDH2 genotype frequency and allele frequency distribution between different sexes(P >0.05).There was no significant difference in ALDH2 genotype frequency and allele frequency between patients younger than 70 years old and those older than 70 years old(both P >0.05).The frequencies of AA genotype and AG genotype in patients younger than 70 years old and those older than 70 years old were lower than the frequency of GG genotype,and the frequency of A allele was lower than the frequency of G allele(all P<0.05).Logistic analysis showed
