单纯性肾囊肿不同手术方式治疗效果的比较分析

摘　　要：目的比较传统三孔、单操作孔及单孔后腹腔镜下行肾囊肿去顶减压术治疗单纯性肾囊肿的临床效果。方法回顾性分析2014年10月至2016年10月,采用腹腔镜治疗单纯性肾囊肿132例。其中传统三孔法72例,单操作孔法34例,单孔法26例,观察3组患者相关临床指标,其中单操作孔法在三孔法的基础上减少了一个操作孔。结果全部手术均顺利完成,无中转开刀病例,术后随访6个月,无明显手术并发症。三组的手术时间分别为(31. 1±17. 4) min、(37. 7±14. 5) min、(50. 5±21. 2) min,单孔组所需手术时间最长,三组的疼痛评分分别为(4. 4±1. 5)、(3. 7±1. 6)、(3. 3±0. 7),传统三孔组获得了最高的疼痛评分,3组患者在术中出血量、术后引流时间、住院时间、术后并发症等方面比较差异无统计学意义下(P>0. 05)。结论单操作孔及单孔法治疗单纯性肾囊肿与传统三孔法相比,疗效相似,而手术创伤小,美容效果好。单操作孔法相较之单孔法,无需特殊装置且更易于掌握,值得临床推广。Objective To compare the clinical effects of traditional three-hole,single-operating hole and single-hole retroperitoneal laparoscopic decompression of renal cysts for the treatment of simple renal cysts.Methods A retrospective analysis of 132 cases of simple renal cysts performed laparoscopic operation from October 2014 to October 2016 was made.Among them,72 cases underwent traditional three-hole method surgery.34 cases were performed single-operating hole operation,and 26 cases were used single-hole method.The clinical indexes of three groups were observed.The single-operating hole method was reduced one operating hole based on the three-hole method.Results All the operations were successfully performed.There were no cases transferred to open surgery.After 6 months of follow-up,there were no obvious complications.The operation time of the three groups was(31.1±17.4)min,(37.7±14.5)min,(50.5±21.2)min.Obviously,the single-hole group required the longest operation time.The pain scores of the three groups were(4.4±1.5).(3.7±1.6),(3.3±0.7).The traditional three-hole group obtained the highest pain score,and the three groups had no difference in intraoperative blood loss,postoperative drainage time,hospitalization time,postoperative complications,etc(P﹥0.05).Conclusion For the treatment of simple renal cysts,single-operating and single-hole methods have similar curative effects compared with the traditional three-hole method,while the surgical trauma is smaller and the cosmetic effect is better.Compared with the single-hole method,the single-operating hole method requires no special device and is easier to grasp,which is worthy of clinical promoting.
