左炔诺孕酮宫内缓释系统在子宫内膜异位症和子宫腺肌病的临床应用研究

摘　　要：目的:观察左炔诺孕酮宫内缓释系统(LNG-IUS)在治疗子宫内膜异位症(EM)和子宫腺肌病(AM)中的临床疗效。方法:选取Ⅰ～Ⅳ期子宫内膜异位症患者20例和子宫腺肌病患者20例作为研究对象,对两组患者进行放置LNG-IUS,观察治疗后EM组和AM组患者的相关疼痛情况、月经状况、子宫状况、血清CA125水平、卵巢功能、肝肾功能、血红蛋白及不良反应发生的情况。结果:治疗后,两组患者相关疼痛情况、月经状况、子宫状况、血清CA125水平和血红蛋白均得到显著改善,差异具有统计学意义(P <0. 05),治疗后两组患者间相关疼痛情况、月经状况、子宫状况和血清CA125水平比较,差异无统计学意义(P> 0. 05);和治疗前比较,两组患者卵巢功能和肝肾功能无显著变化,差异无统计学意义(P> 0. 05)。结论:左炔诺孕酮宫内缓释系统(LNG-IUS)在子宫内膜异位症(EM)和子宫腺肌病(AM)的临床疗效显著,且简便、安全,值得临床推广。Objective To study the clinical effect of levonorgestrel-releasing intrauterine system on pain and recurrence associated with endometriosis and adenomyosis.Method 40 patients who were selected were randomly divided into EM group and AM group,20 cases in each group.The two groups were given LNG-IUS treatment.Pain situation,menstruation,uterine conditions,CA125 levels,ovarian function,hepatorenal function,hemoglobin and side-effect were observed after treatment between two groups.Results After treatment pain situation,menstruation,uterine conditions,CA125 levels and hemoglobin in EM group and AM group were improved obviously(P<0.05),the differences of above parameters between two groups were not statistically significant(P >0.05).After treatment ovarian function and hepatorenal function in EM group and AM group were not improved obviously(P >0.05),the differences of above parameters between two groups were not statistically significant(P >0.05).Conclusion Levonorgestrel-releasing intrauterine system can effectively improve the symptoms of adenomyosis,and help to control the progression of EM and AM,which is safe and effective,worthy of further study.
