钬激光前列腺剜除术与经尿道前列腺电切术治疗良性前列腺增生的临床对比分析

摘　　要：目的对比钬激光前列腺剜除术(HoLEP)与经尿道前列腺电切术(TURP)治疗良性前列腺增生(BPH)的临床疗效。方法将86例BPH患者随机分为两组,分别采用TURP和HoLEP方法,并对术中及术后指标进行统计和分析。结果两组患者手术前后的前列腺症状评分(IPSS)、最大尿流率(Q_(max))、残余尿(PVR)均明显改善,差异具有统计学意义(P<0.05);且HoLEP组具有术中出血少,术后膀胱冲洗时间、留置导尿时间、住院时间短、并发症低的特点。结论 HoLEP与TURP治疗BPH效果相近,并具有术中、术后出血更少,术后恢复时间短的优点,值得临床应用和推广。Objective To compare the clinical efficacy of Holmium laser enucleation of the prostate(HoLEP)and transurethral resection of the prostate(TURP)in the treatment of benign prostatic hyperplasia.Methds 86 elderly male BPH patients who underwent surgical treatment were randomly divided into TURP group and HoLEP group.The preoperative,intraoperative and postoperative conditions of the two groups were compared.Results There was n significant difference between the two groups before operation;in HoLEP group,there was less bleeding during operation,less bladder irrigation time,shorter indwelling catheterization time,shorter hospital stay and lower complications after operation;IPSS,Qmax and PVR were significantly improved after operation compared with those before operation,and there was no significant difference between the two groups.Conclusions Holmium laser enucleation of the prostate is similar to transurethral resection of the prostate in the treatment of benign prostatic hyperplasia.HoLEP has less bleeding and shorter recovery time,which is worthy of clinical promotion.
