偏头痛患者长期服用散利痛导致不射精症1例报告与文献复习

摘　　要：目的增补1例长期使用非甾体类解热镇痛药导致不射精症的药物使用空白,并对文献进行复习,以引起临床工作者的注意。方法系统回顾1例偏头痛患者长期服用散利痛导致器质性不射精症的详细病史和诊疗经过。结果由于患者对药物的依赖而无法停药,尝试了许多男科治疗手段,均以失败告终。结论散利痛导致不射精的机理可能与过高的血药浓度干扰了5-HT对射精中枢某个通路的调控作用。不射精症治疗方式多样,但总体有效率低,自然受孕机率低,最终可能需要通过睾丸取精或输精管取精技术获取精子,完成辅助生殖。Objective To introduce a case with migraine resulting into anejaculation due to long-term using non-steroidal anti-inflammatory drugs,and to call the attention of clinicians through literature review.Methods The detailed medical history,diagnosis and treatment of a migraine patient who got anejaculation due to long-term using Saridon.Results Administration of Saridon could not be stopped due to patients'dependence on the drug,and a lot of measures taken in the andrology department ended in failure.Conclusions The mechanism of anejaculation caused by Saridon is probably that too high plasma concentration interfered with the regulation of 5-HT on a certain pathway in the ejaculation center.Though there are many treatments for anejaculation,the overall efficiency and rate of natural conception is low.It may be necessary to obtain sperm by testicular spermatozoa or vas deferens sperm retrieval techniques to complete assisted reproduction.
