LigaSure血管闭合系统在经腹宫颈癌根治术中的应用价值分析

摘　　要：目的评价在经腹宫颈癌根治术中应用Liga Sure血管闭合系统的疗效与安全性。方法选择2015年6月至2016年6月间于珠海市第二人民医院进行经腹宫颈癌根治术的Ia～IIa期患者48例,随机分为Liga Sure组(24例)和常规组(24例),在经腹广泛子宫切除术的基础上,分别使用Liga Sure血管闭合系统和常规钳夹/结扎进行手术止血,术后随访180天,比较两组患者手术时间、术中失血量、输血率、术前和术后血红蛋白及红细胞压积、盆腔淋巴结清扫情况、术后疼痛程度、住院时间和术后并发症(出血、感染、尿潴留和淋巴囊肿)。结果 Liga Sure组与常规组的手术时间分别为(249. 6±43. 2) min和(340. 4±81. 7) min,术中失血量分别为(588. 2±286. 6) m L和(978. 2±324. 2) m L,Liga Sure组的输血率、术后疼痛程度、手术时间和并发症发生率显著低于常规组,Liga Sure组术后血红蛋白、红细胞压积和盆腔淋巴结清扫数目显著高于常规组,以上差异间均具有统计学意义(P <0. 05)。结论与传统结扎止血方式相比,在经腹宫颈癌根治术中应用Liga Sure血管闭合系统可以显著缩短手术时间,减少术中出血量,减轻患者术后疼痛程度,降低发生术后并发症的风险。Objective To observe the efficacy of LigaSure vessel sealing system in transabdominal radical operation of cervical carcinoma.Methods 48 patients with Ia^IIa cervical carcinoma treated by transabdominal radical operation(24 cases in LigaSure group and 24 cases in conventional group)in our hospital from June 2015 to June 2016 were selected.LigaSure vessel sealing system(Valley lab,US)was used in LigaSure group,and conventional suture was used in conventional group.The operation time,hospitalization days,blood loss,blood transfused rate,preoperative and postoperative hematocrit/hemoglobin,number of pelvic lymph nodes dissected,postoperative pain level and complications(such as bleeding,infection,uroschesis and lymphocyst)were comparatively analyzed.Results The operation time was(249.6±43.2)min in LigaSure group and(340.4±81.7)min in conventional group,and blood loss was(588.2±286.6)mL in LigaSure group and(978.2±324.2)mL in conventional group.The post-operative hematocrit,hemoglobin level and number of pelvic lymph nodes dissected in LigaSure group were significantly higher than those of conventional group.The hospitalization days,blood transfused rate,postoperative pain level and complications rate in LigaSure group were significantly lower than those of conventional group,with statistically significant differences(P<0.05).Conclusions Compared with traditional ligation and hemostasis,LigaSure vessel sealing system in transabdominal radical operation of cervical carcinoma can significantly shorten operation time,reduce intraoperative blood loss,improve postoperative pain and reduce the risk of postoperative complications.
