持续性剖宫产瘢痕妊娠的临床特点分析

摘　　要：目的:比较持续性剖宫产瘢痕妊娠(PCSP)与普通剖宫产瘢痕妊娠(NCSP)的临床特点及治疗方法差异。方法:收集2014年12月—2017年12月在本院诊治的剖宫产瘢痕妊娠(CSP)89例临床资料,其中PCSP 15例、NCSP 74例,比较两组一般情况及诊治情况。结果:两组剖宫产次数、距离前次剖宫产时间比较无差异(P>0.05);与NCSP组相比,PCSP组年龄、流产次数、停经天数、子宫下段切口处包块均较大,瘢痕厚度更薄、III型CSP占比更高,血hCG恢复时间更快(均P<0.05);治疗中PCSP行经腹瘢痕切除修复术的比率高于NCSP组(P<0.001)。结论:PCSP的患者更易发生III型CSP,且手术干预几率增加,须引起临床医生重视,CSP的早诊断和初次成功治疗可降低PCSP的发生。Objective:To compare the difference of clinical features and therapeutic methods between women with persistent cesarean scar pregnancy(PCSP)and women with normal cesarean scar pregnancy(NCSP).Methods:The data of 89 women with cesarean scar pregnancy(CSP)from December 2014 to December 2017 were retrospective analyzed,and these 89 women were divided into PCSP group(15 cases)and NCSP group(74 cases).The differences of general situation and clinical therapeutic of women were compared between the two groups.Results:The times of cesarean and the interval time from the last cesarean of women had no statistically significance different between the two groups(P>0.05).Compared to those of women in NCSP group,the women in PCSP group were older,their abortion times was more,their average menopause days was longer,lump diameter of scar was longer,scar was thinner,the rate of III CSP was higher,and the time of resume HCG level recovered to normal was shorter(P<0.05).The rate of needing laparotomy resection and repair of the cesarean scar of women in PCSP group was significant higher than that of women in NCSP group(P<0.001).Conclusion:The women with PCSP are more likely to develop to III CSP,and the probability of needing surgical treatment is increased,which should be paid more attention to by clinicians.The diagnosis early and initial treating CSP successfully can reduce the incidence of PCSP.
