摘 要:目的探讨心脏超声在慢性心力衰竭(CHF)患者诊断及心脏功能评价中的作用。方法选取我院2015年1月~2018年1月诊疗的137例CHF患者为观察组,同期健康体检人员137例为对照组,均行心脏超声检查和心脏功能评价。结果观察组左室射血分数(LVEF)、心脏指数(CI)低于对照组;左室舒张末期内径(LVEDD)、左房内径(LV)、HFEI评分高于对照组(P<0.05)。心功能Ⅳ级患者LVEF、CI低于心功能Ⅰ、Ⅱ、Ⅲ级患者;LVEDD、LV、HFEI评分高于心功能级Ⅰ、Ⅱ、Ⅲ级患者(P<0.05)。心功能Ⅲ级患者LVEF、CI低于心功能Ⅰ、Ⅱ级患者;LVEDD、LV、HFEI评分高于心功能Ⅰ、Ⅱ级患者(P<0.05)。心功能Ⅱ级患者LVEF、CI低于心功能Ⅰ级患者;LVEDD、LV、HFEI评分高于心功能Ⅰ级患者(P<0.05)。心脏功能分级与LVEF、CI成正相关性(P<0.05);与LVEDD、LV、HFEI评分成负相关性(P<0.05)。结论心脏超声可用于CHF的临床诊断,与心脏功能评价密切相关。Objective To investigate effect of echocardiography in diagnosis and cardiac function evaluation for patients with chronic heart failure(CHF). Methods A total of 137 patients with CHF were selected in our hospital from January 2015 to January 2018 as observation group. 137 health checkup people were selected at same period as control group. All people received echocardiography and cardiac function evaluation. Results Left ventricular ejection fraction(LVEF), cardiac index(CI) in the observation group were lower than that in the control group;while left ventricular end diastolic diameter(LVEDD), left atrial diameter(LV), HFEI score in the observation group were higher than those in the control group (P<0.05). LVEF, CI in patients with grade Ⅳ were lower than that patients with grade Ⅰ,Ⅱ,Ⅲ;while LVEDD, LV, HFEI scores in patients with grade Ⅳ were higher than those patients with grade Ⅰ,Ⅱ,Ⅲ(P<0.05). LVEF, CI in patients with grade Ⅲ were lower than those patients with gradeⅠ,Ⅱ;while LVEDD, LV, HFEI scores in patients with grade Ⅲ were higher than those patients with grade Ⅰ,Ⅱ(P<0.05). LVEF, CI in patients with grade Ⅱwere lower than those patients with grade Ⅰ;while LVEDD, LV, HFEI score in patients with grade Ⅱwere higher than those patients with grade I (P<0.05). There were positive correlation between cardiac function classification and LVEF, CI (P<0.05). There were negative correlation between cardiac function classification and LVEDD, LV, HFEI score (P<0.05). Conclusion Echocardiography can be used in clinical diagnosis of CHF, which is closely related to cardiac function evaluation.
