基于集聚度的江苏省卫生资源配置公平性分析

摘　　要：目的对江苏省2011-2016年的卫生资源配置的公平性从地理、人口和经济三方面进行分析,促进其更公平、有效的卫生资源配置。方法引用集聚度研究理论,计算江苏省卫生资源集聚度、人口集聚度和经济集聚度并进行统计分析。结果 (1)从地理配置层面看,除卫生机构数外的其余指标的地理集聚度均在全国平均水平的5倍左右,表明江苏省的卫生资源相对于全国较为集中,从地理配置层面上来说相对过剩;(2)从人口配置层面看,江苏省卫生人力资源基本上满足其人口集聚度的要求,而卫生机构数的HRADi/PADi的比值近年来只略大于0. 5,其按人口配置显得较为不足,表明较多的卫生资源集中于较少的卫生机构数,容易导致看病难的问题;(3)从经济集聚度来看,江苏省占全国国民生产总值的比重较大,但是卫生资源中经济占比却均<1,表明江苏省对卫生资源经济投入上的不足。结论政府应鼓励多元办医,优化卫生机构的资源分布和空间分布,加强财政支持,提升卫生人力、物力资源的经济投入,兼顾多种因素以促进江苏省卫生资源更合理、有效配置。Objective:To analyze the fairness of health resource allocation from 2011 to 2016 in the 3 aspects of geography,population and economy in Jiangsu Province,and promote the more equitable and effective health resource allocation.Method:The authors calculated the concentration of health resources,population and economy with the theory of agglomeration degree and made the statistical analysis.Result:Geographically,the geographical concentration of indicators is 5 times that of national average,except the number of health institutions,which shows that health resources in Jiangsu Province are more concentrated than those in the whole country.From the perspective of population distribution,health human resources in Jiangsu Province basically meet the requirement of population concentration.However,the ratio of HRADi/PADi of the number of health institutions is only slightly larger than 0.5 in recent years,which is relatively inadequate according to population configuration.It shows that more health resources are concentrated in fewer health institutions,which easily leads to the difficulty in seeing a doctor.From the degree of economic aggregation,although Jiangsu Province accounts for a larger proportion of the gross national product in the whole country,the economic proportion of health resources is<1,which shows the inadequate investment in health resources in Jiangsu.Conclusion:The government should encourage diversified hospital operation,optimize resource distribution and space distribution of health institutions,enhance financial support,input more in health human resources,and material resources,take account of various factors so as to promote more rational and effective allocation of health resources.
