摘　　要：目的评估GeneXpert Mtb/RIF检测在结核病快速诊断中的应用价值。方法筛选我院肺科门诊120例初诊疑似结核病的患者,取晨痰标本,进行痰涂片镜检、罗氏培养和GeneXpert Mtb/RIF快速检测,GeneXpert Mtb/RIF与罗氏培养均为阳性标本进行药敏检测,最终评估GeneXpert Mtb/RIF检测的敏感度和特异度以及Kappa值。结果 120例有效标本中,阳性检测率涂片镜检、罗氏培养、GeneXpert Mtb/RIF检测分别为25.8%、44.7%、46.7%;涂片镜检、GeneXpert检测敏感度、特异度分别为78.33%(94/120)、75.8%(91/120);98.33%(118/120)、99.16%(119/120)。结论 GeneXpert Mtb/RIF检测在疑似结核病诊断中较快速,准确,利于结核病的早期快速诊断。Objective To evaluate the value of GeneXpert Mtb/RIF in rapid diagnosis of suspected TB patients. Methods 120 with suspected tuberculosis in pulmonary outpatient department of our hospital were selected.The samples of morning sputum were collected at the first visit, the sputum smears were examined by microscope,GeneXpert Mtb/RIF was detected quickly, and the drug sensitivity was detected in the positive samples of GeneXpert Mtb/RIF and L-J culture.The sensitivity,specificity and Kappa value of GeneXpert Mtb/RIF were evaluated. Results 120 The positive rate of smear examination, L-J culture and GeneXpert were 25.8%,44.7% and 46.7%,respectively.The sensitivity and specificity of GeneXpert were 78.33%(94 / 120), 75.8%(91 / 120),98.33%(118 / 120) and 99.16%(119 / 120) respectively. Conclusion GeneXpert Mtb/RIF detection is rapid and accurate in the diagnosis of suspected patients with tuberculosis.
