血清H-FABP与风湿性心脏病患者心功能的关系分析

摘　　要：目的探讨血清心型脂肪酸结合蛋白(H-FABP)与风湿性心脏病(RHD)患者心功能的相关关系。方法选取2016年1月至2018年1月于阜阳市人民医院收治的81例RHD患者为实验组,同期选取50例常规体检的心功能正常者作为对照组,比较两组受试者及不同美国纽约心脏病协会心功能分级RHD患者的血清H-FABP、B型钠尿肽前体N端肽(NT-proBNP)、肌钙蛋白T(cTnT)水平及左心室舒张末期内径(LVEDD)、左心室射血分数(LVEF),并分析RHD患者血清H-FABP与以上各指标的相关性。结果实验组血清NT-proBNP、cTnT、H-FABP水平及LVEDD均较对照组高,而LVEF比对照组低,差异具有统计学意义(P均<0.05)。RHD患者中NT-proBNP、cTnT、H-FABP、LVEDD水平随着心功能分级的增加而升高,而LVEF水平则相反,组间比较差异有统计学意义(P均<0.05)。血清H-FABP与NT-proBNP、cTnT、LVEDD及NYHY心功能分级呈正相关(r=0.721,0.672,0.641,0.714,P<0.05),而与LVEF呈负相关(r=-0.613,P<0.05)。结论 RHD患者血清H-FABP水平异常升高,且与患者心功能成负相关,临床可以通过检测H-FABP水平评估患者心功能。Objective To study the relationship between serum heart fatty acid binding protein(H-FABP)and cardiac function in patients with rheumatic heart disease(RHD).Methods 81 RHD patients admitted in Fuyang City People's Hospital from January 2016 to January 2018 were selected as study group,meanwhile another 50 healthy persons with normal cardiac function were selected as control group.The serum level of H-FABP,N-terminal fragment of the prohormone brain-type natriuretic peptide(NT-proBNP),troponin t(cTnT),left ventricular end-diastolic diameter(LVEDD)and left ventricular ejection fraction(LVEF)was compared between the two groups and RHD patients with different NYHA grades,and the correlation between H-FABP and the above indexes was analyzed.Results The level of serum NT-proBNP,H-FABP,cTnT and LVEDD in the study group was significantly higher than those in the control group(P<0.05),but the level of LVEF was lower than that in the control group(P<0.05).The level of serum NT-proBNP,H-FABP,cTnT and LVEDD increased with the increase of cardiac function grading,while LVEF level was opposite,there was a significant difference between the two groups(P<0.05).Serum H-FABP was positively correlated with NT-proBNP,cTnT,LVEDD and NYHY(r=0.721,0.672,0.641,0.714,P<0.05),but negatively correlated with LVEF(r=-0.613,P<0.05).Conclusion The level of serum H-FABP in patients with RHD is abnormally elevated and is negatively correlated with cardiac function.The level of H-FABP can be measured to evaluate the cardiac function of patients with RHD.
