胫骨高位截骨联合关节镜下清理术治疗膝关节内侧单间室骨性关节炎的短期疗效观察

摘　　要：目的研究胫骨高位截骨联合关节镜下清理术治疗膝关节骨性关节炎的短期临床疗效。方法选取2018年3月～2018年10月在我院采用联合手术治疗早期膝关节骨性关节炎并膝内翻畸形的患者12例(共12膝)作为研究对象,随访时间2～3个月,进行回顾性分析,对患者术前与术后1月、2月的股胫角(FTA)、胫骨近端内侧角(MPTA)、膝关节内侧间隙、VAS评分和HSS评分对比分析,对术后并发症发生情况进行记录分析。结果所有经过胫骨高位截骨联合关节镜下清理术治疗的患者术后均未发生皮肤坏死、切口愈合不良、术后感染等并发症,患者术前、术后1月、术后2月的FTA、MPTA和膝关节内侧间隙的平均值依次分别为(188.5±3.8)°、(82.3±4.1)°、(1.9±0.6)mm;(173.8±2.1)°、(86.1±2.6)°、(4.6±1.2)mm;(173.6±1.8)°、(86.3±1.9)°、(4.7±1.4)mm;其VAS评分及HSS评分均较术前明显改善,差异有统计学意义(P<0.05)。结论胫骨高位截骨联合关节镜下清理术对于早期膝关节骨性关节炎并膝内翻畸形的患者短期疗效较好,值得临床推广。Objective To study the short-term clinical effect of high tibial osteotomy combined with arthroscopic debridement in the treatment of knee osteoarthritis. Methods From March 2018 to October 2018, 12 patients with early knee osteoarthritis and varus deformity (12 knees in total, followed up for 1 to 2 months) were retrospectively analyzed. The VAS score and HSS score of femoral tibial angle (FTA), proximal medial tibial angle (MPTA) and medial knee space before and after operation were compared and analyzed. The occurrence of the disease was recorded and analyzed. Results All patients treated with high tibial osteotomy combined with arthroscopic debridement had no complications such as skin necrosis, poor wound healing and postoperative infection. The average of FTA angle, MPTA angle and medial knee joint of patients before surgery, 1 month and 2 months after surgery were(188.5±3.8)°、(82.3±4.1)°、(1.9±0.6)mm;(173.8±2.1)°、(86.1±2.6)°、(4.6±1.2)mm;(173.6±1.8)°、(86.3±1.9)°、(4.7±1.4)mm, respectively. The VAS and HSS scores were significantly improved compared with preoperative ones, and the difference was statistically significant( P < 0.05). Conclusion High tibial osteotomy combined with arthroscopic debridement has a good short-term effect on early knee osteoarthritis with varus deformity, which is worthy of clinical promotion.
