两种术式治疗脊椎骨折的效果对比观察

摘　　要：目的对比经皮微创与开放式椎弓根钉内固定术治疗脊椎骨折的效果。方法选取2016年3月至2018年3月本院收治的52例脊椎骨折患者为本次研究对象,随机分为研究组和对照组,各26例,对照组采取开放式椎弓根钉内固定术治疗,研究组则采取经皮微创椎弓根钉内固定术治疗,比较两组手术情况(术中出血量、手术时间、切口长度)、住院时间、术后Cobb's角、椎体前缘高度变化,观察并发症发生情况。结果研究组术中出血量少于对照组,切口长度、手术时间、住院时间均短于对照组,比较差异有统计学意义(P<0.05);而术后研究组Cobb's角、椎体前缘高度及血清肌酸激酶活性和对照组相比差异无统计学意义;研究组并发症发生率低于对照组,比较差异有统计学意义(P<0.05)。结论两种术式均能达到较好治疗脊椎骨折的效果,两者差异不大,但经皮微创术式手术情况较开放式手术好,更有利于患者预后。Objective To compare the effects of percutaneous minimally invasive and open pedicle screw fixation in the treatment of spinal fractures. Methods A total of 52 patients with spinal fractures admitted to our hospital from March 2016 to March 2018 were selected for the study and randomly divided into study group and control group, with 26 cases in each group. Control group was treated with open pedicle screw fixation, and study group was given percutaneous minimally invasive pedicle screw fixation. The operation conditions (intraoperative blood loss, operative time, incision length), hospital stay, postoperative Cobb’s angle and anterior vertebral height were compared between the two groups. And the occurrence of complications was observed. Results The intraoperative blood loss in study group was lower than that in control group, and the incision length, operative time and hospital stay were shorter than those in control group (P<0.05). There was no significant difference in the postoperative Cobb’s angle and anterior vertebral height and serum creatine kinase activity in study group compared with those in control group. The incidence rate of complications in study group was lower than that in control group (P<0.05). Conclusion The two surgical methods can achieve good treatment effects of spinal fractures. And the difference between the two is small, but the percutaneous minimally invasive surgery is better than the open surgery, and it is more conducive to the prognosis.
