尿石清合剂对湿热型泌尿系结石患者肾盂压力、炎症指标、尿量及钙离子代谢影响的研究

摘　　要：目的:探讨尿石清合剂对湿热型泌尿系结石患者肾盂压力、炎症指标(PCT、CRP)、尿量及血清Ca^(2+)离子代谢的影响。方法:选择中山市中医院收治的湿热型泌尿系结石患者180例,全部给予手术治疗,每组60例。术后观察组给予尿石清合剂及头孢呋辛脂片,对照组给予尿石通丸及头孢呋辛脂片,空白组予头孢呋辛脂片,比较三组患者术后肾盂压力、PCT、CRP、尿量及血清Ca^(2+)离子的变化。结果:观察组治疗后肾盂压力为(12.9±2.2)mmHg,显著高于对照组及空白组(P<0.05)。治疗后观察组PCT下降至(0.52±0.21)ng/ml、CRP下降至(10.22±1.56)mg/L,显著高于对照组及空白组。观察组治疗后尿量(2705±685)ml,显著高于对照组及空白组(P<0.05)。治疗后三组患者Ca^(2+)均有降低,观察组患者显著低于对照组及空白组(P<0.05)。结论:经皮肾镜取石术后加服尿石清合剂可显著提高肾盂压力,降低炎症指标,增加尿量,降低血清钙Ca^(2+)离子水平,具有清热祛湿,解毒通淋,利尿排石的功效,临床疗效较好。Objective:To discuss the clinical effects of NiaoShi Qing mixture on shi re type patients with renal pelvis pressure,inflammation,urine output and calcium metabolism.Methods:180 patients with lithangiuria from Zhongshan hospital of traditional Chinese medicine.The observation group were treated with Niaoshiqing mixture and cefuroxime axetil tablet,the control group were treated with Niaoshitong grain and cefuroxime axetil tablet,the blank group were treated with cefuroxime axetil tablet,then postoperative renal pelvic pressure,the changes of inflammation,urine output and calcium metabolism before and after treatment were contrasted between the three groups.Results:The postoperative renal pelvic pressure in the observation group was (12.9±2.2) mmHg,significantly higher than the control group and the blank group (P<0.05),after treatment.The PCT in the observation group decreased to(0.52±0.21)ng/ml、CRP decreased to(10.22±1.56)mg/L,significantly higher than the control group and blank group,urine volume after the treatment in the observation group was (2705±685) ml,significantly higher than the control group and the blank group (P<0.05).Ca^2+ was decreased in all three groups.After treatment,and the observation group significantly lower than the other two groups(P<0.05).Conclusion:Niaoshiqing mixture after percutaneous nephroscope can significantly improve the renal pelvic pressure,reduce inflammation index,increase urine volume,reduce serum Ca^2+ level,has effects in clearing away heat and dampness,removing toxic materials,relieving stranguria,promoting urination,and removing calculus,The clinical curative effect is remarkable.
